      目的 探讨2型糖尿病(T2DM)患者三酰甘油-葡萄糖(TyG)指数与骨转换标志物及骨密度之间的相关性,分析胰岛素抵抗对骨质疏松症的作用。方法 回顾性选取2017年1月至2022年12月在中国中医科学院广安门医院内分泌科住院治疗的T2DM成年患者病历资料,收集患者空腹葡萄糖(FBG)、三酰甘油(TG)计算TyG指数,收集Ⅰ型胶原C-末端肽交联(CTX)、Ⅰ型原胶原N-端前肽(P1NP)、骨钙素(OC)、血清碱性磷酸酶(ALP)等骨转换标志物和骨密度(BMD)。根据TyG指数四分位数分组比较不同组间差异,应用Spearman相关性分析、多元线性回归等分析TyG指数与骨质疏松症的相关性。结果 最终纳入患者4 541例。Spearman相关性分析显示,TyG指数与CTX、P1NP、OC呈负相关(P<0.05),与ALP呈正相关(P<0.05)。多元线性回归显示,TyG指数与CTX、OC呈负相关(P<0.05)。广义加性模型显示,TyG指数与P1NP存在非线性关系(P<0.05)。调整其他影响因素后的TyG指数与ALP无关(P>0.05)。结论 T2DM患者较高的TyG指数与低骨转换有关,TyG指数是T2DM患者发生骨质疏松症的重要预测指标。

      Objective To explore the correlation between the triglyceride-glucose(TyG) index and bone turnover markers as well as bone mineral density(BMD) in patients with type 2 diabetes,and to analyze the role of insulin resistance in osteoporosis.Methods Retrospective of medical records of T2DM adult patients admitted to the Endocrinology Department of Guang′anmen Hospital,Chinese Academy of Chinese Medical Sciences from January 2017 to December 2022 were selected.Data on fasting blood glucose(FBG) and triglycerides(TG) was collected to calculate the TyG index.Bone turnover markers such as C-terminal telopeptide of type I collagen(CTX),N-terminal propeptide of type I procollagen(P1NP),osteocalcin(OC),and serum alkaline phosphatase(ALP),as well as BMD,were recorded.Patients were grouped according to the quartiles of the TyG index to compare the differences among groups.Spearman correlation analysis and multiple linear regression were used to analyze the association between the TyG index and osteoporosis.Results A total of 4 541 patients were finally included.Spearman correlation analysis showed that the TyG index was negatively correlated with CTX,P1NP,and OC(P<0.05),and positively correlated with ALP(P<0.05).After adjusting for other influencing factors,multiple linear regression revealed a significant negative correlation between the TyG index and CTX as well as OC(P<0.05),while the generalized additive model indicated a nonlinear relationship between the TyG index and P1NP(P<0.05).After adjustment for other factors,the TyG index showed no association with ALP(P>0.05).Conclusions A higher TyG index in patients with type 2 diabetes is associated with lower bone turnover,suggesting that the TyG index is an valuable predictive indicator for the development of osteoporosis in patients with type 2 diabetes.
