| 目的 通过聚类分析探索中央型气道恶性肿瘤患者的临床特征,总结其证候特点。方法 采取回顾性研究设计,对2020年3月至2021年10月北京中医药大学东直门医院呼吸病中心收治的中央型气道恶性肿瘤患者进行研究。选取符合纳入标准的患者共152例,收集患者的临床症状、体征及其他临床信息,聚类分析总结中央型气道恶性肿瘤的临床特征和证候特点。结果 将35个高频次症状分为4类,代表不同证候类型:第一类,肺脾气虚兼痰湿；第二类,肺、脾、肾虚兼痰热；第三类,肺、脾、肾虚兼阴伤；第四类,肺、脾、肾虚日久。结论 中央型气道恶性肿瘤患者的主要临床特征是肺、脾、肾三脏虚兼见痰湿、痰热及阴伤,基本病机是虚实夹杂、正虚邪积。 |

| Objective To explore the clinical characteristics of patients with central airway malignant tumors and summarize their syndrome characteristics by cluster analysis.Methods A retrospective study design was adopted to investigate the patients with central airway malignant tumors admitted to Dongzhimen Hospital of Beijing University of Chinese Medicine from March 2020 to October 2021.A total of 152 patients who met the inclusion criteria were selected,and the clinical symptoms,signs and other clinical information of the patients were collected.Cluster analysis was used to summarize the clinical characteristics and syndrome characteristics of central airway malignant tumors.ResultsThirty-five symptoms with high frequency were divided into four categories.The first category,deficiency of lung and spleen combined with phlegm dampness;the second category,deficiency of lung,spleen and kidney combined with phlegm heat;the third category,deficiency of lung,spleen and kidney combined with Yin injury;the fourth category,chronic deficiency of lung,spleen and kidney.Conclusions The main clinical characteristics of patients with central airway malignancies are deficiency of lung,spleen and kidney with phlegm-dampness,phlegm-heat,consumption of yin fluid.The basic pathogenesis is combination of excess syndrome and deficiency syndrome. |
