| 目的 探讨预后营养指数(PNI)术前评估胆管细胞癌(CCA)对预后判断的临床意义。方法 回顾性分析中国人民解放军总医院第一医学中心13年间经病理确诊的384例CCA根治性手术患者病历资料。利用Kaplan-Meier法绘制生存曲线、Log-rank计算P值进行单因素分析,绘制受试者工作特征(ROC)曲线判断PNI预后评估效能,并用多元Cox回归法进行多因素分析。结果 预测2年总生存期最佳PNI截断值为46.48,最高Youden指数为0.26,灵敏度为49.4%,特异性为76.6%,曲线下面积为0.65。结论 PNI是CCA根治性手术患者预后评估的独立因素。PNI术前评估有助于对CCA患者预后判断。 |

| Objective To explore the clinical significance of preoperative evaluation of prognostic nutritional index(PNI) for cholangiocarcinoma (CCA) in predicting prognosis.Methods The medical records of 384 CCA patients diagnosed by radical surgery and pathology in the First Medical Center of PLA General Hospital in 13 years were retrospectively analyzed.Kaplan-Meier method to plot survival curves and Log-rank were used to calculate P values for univariate analysis,receiver operating characteristic(ROC) curves to assess the prognostic efficacy of PNI,multivariate Cox regression for multivariate analysis.Results The optimal PNI threshold for predicting 2-year overall survival was 46.48,the highest Youden index was 0.26,sensitivity was 49.4%,specificity was 76.6%,and the area under the curve was 0.65.Conclusions PNI is an independent prognostic factor for CCA radical surgery.Preoperative evaluation of PNI is helpful for predicting the prognosis of CCA patients. |
