肌少症是一种常见的老年综合征,如不及时干预极易发展为衰弱和失能,会严重影响患者的生活质量和健康预期寿命。目前,肌少症的干预策略以营养支持和运动疗法为核心,尚缺乏针对性的有效药物。β-羟基-β-甲基丁酸(HMB)作为亮氨酸的代谢产物,通过抗分解代谢、促进蛋白质合成、减轻氧化应激和炎症反应等多重机制,对老年肌少症患者的骨骼肌产生一定的保护甚至“增肌”作用。HMB作为膳食补充剂和口服营养补充剂的添加剂已广泛应用多年,但其对于肌少症的疗效及作用机制仍需进一步深入研究。该文对HMB的作用机制及其在老年肌少症患者中的临床研究进展进行综述,为临床合理应用提供参考依据。 |

Sarcopenia is a common geriatric syndrome that,if not intervened in a timely manner,can easily progress to frailty and disability,significantly impairing patients′ quality of life and healthy life expectancy.Currently,intervention strategies for sarcopenia primarily focus on nutritional support and exercise therapy,due to the lack of targeted and effective pharmacological interventions.Beta-hydroxy-beta-methyl butyrate (HMB),a metabolite of leucine,has shown notable protective and even "muscle-building" effects on skeletal muscle in elderly patients with sarcopenia.These benefits are mediated through multiple mechanisms,including anti-catabolism,promotion of protein synthesis,and reduction of oxidative stress and inflammatory responses.Although HMB has been widely used as a dietary supplement and an additive in oral nutritional supplements for many years,its specific efficacy and mechanisms in sarcopenia still require further in-depth research.This article reviews the mechanism of action of HMB and its clinical research progress in elderly patients with sarcopenia,providing reference for its rational clinical application. |
