      目的 探讨尿碘(UIC)与尿碘肌酐比(UI/Cr)和甲状腺疾病的相关性。方法 该研究是前瞻性研究。选取2022年12月至2023年12月安徽医科大学附属省立医院的健康体检者和内分泌科住院患者为研究对象,共446例。所有人进行血清甲状腺功能、甲状腺抗体、UIC、UI/Cr检测及甲状腺彩色多普勒超声检测。根据甲状腺功能及彩色多普勒超声结果将研究对象分为:对照组、甲亢组、结节组、甲亢合并结节组。采用非参数检验及单因素方差分析法比较4组间的一般数据,以及UIC和UI/Cr水平差异。应用Spearman相关法分析4组间UIC及UI/Cr与不同甲状腺功能评价指标的相关性,同时采用二元logistic回归分析UIC、UI/Cr对甲状腺功能指标的影响,以UIC分层分组进行多元线性回归分析UI/Cr与甲状腺功能的关系。结果 甲亢合并结节组UIC水平显著高于对照组及结节组,结节组UI/Cr显著高于对照组、甲亢组。根据Spearman相关性分析,甲状腺功能亢进症患者中的UI/Cr与游离三碘甲状腺原氨酸(FT3)及游离甲状腺素(FT4)的浓度显著正相关(P<0.05),结节组UIC、甲亢合并结节组UI/Cr与FT3水平呈正相关(P<0.05)。二元logistic回归分析发现,UIC水平(OR=1.986,95%CI:1.384~2.848,P<0.001)是患甲状腺功能亢进症的危险因素,UI/Cr水平(OR=1.002,95%CI:1.000~1.003,P=0.022)是患甲状腺结节的危险因素。结论 UIC、UI/Cr可用于甲状腺疾病碘营养状况的评估。高碘是甲状腺功能亢进症患者患病的危险因素,同时碘过量摄入可能会增加甲状腺结节的患病率。

      Objective To investigate the relationship between urinary iodine (UIC),urinary iodine-creatinine ratio (UI/Cr) and thyroid diseases.Methods This study was a prospective study,selecting 446 people were selected from healthy examinees and inpatients from Anhui Provincial Hospital Affiliated to Anhui Medical University from December 2022 to December 2023 as the research subjects.According to thyroid function and color ultrasound results,the subjects were divided into control group,hyperthyroidism group,nodule group and hyperthyroidism with nodule group.Non-parametric test and one-way analysis of variance were used to compare the general data,UIC and UI/Cr levels among the four groups.The Spearman correlation method was utilized to assess the relationship between UIC and UI/Cr and different thyroid function evaluation indexes among the four groups,binary logistics regression was used to analyze the effects of UIC and UI/Cr on thyroid function indexes,and multiple linear regression was used to analyze the relationship between UI/Cr and thyroid function by hierarchical grouping of UIC.Results The UIC level of hyperthyroidism combined with nodule group was significantly higher than that of control group and nodule group,and UI/Cr of nodule group was significantly higher than that of control group and nodule group.Spearman correlation analysis indicated a positive correlation between the UI/Cr and levels of FT3 as well as FT4 in the hyperthyroidism group (P<0.05).Additionally,UIC in nodule group and UI/Cr hyperthyroidism combined with nodules group was also positively correlated with FT3 levels(P<0.05).Binary logistics regression analysis showed that UIC level(OR=1.986,95%CI:1.384-2.848,P<0.001)was a risk factor for hyperthyroidism.UI/Cr level(OR=1.002,95%CI:1.000-1.003,P=0.022) was a risk factor for thyroid nodules.Conclusions UIC and UI/Cr can be used to evaluate iodine nutrition in thyroid diseases.High iodine is a risk factor for hyperthyroidism,and excessive iodine intake may increase the prevalence of thyroid nodules.
