| 目的 探讨亚麻醉剂量艾司氯胺酮对老年女性患者无痛胃镜检查后睡眠质量的影响。方法 该研究为前瞻性研究。选取2024年7月至9月在安徽医科大学第一附属医院接受无痛胃镜检查的老年女性患者80例,用随机数字表法将患者分为亚麻醉剂量艾司氯胺酮复合依托咪酯组(观察组,40例)和舒芬太尼复合依托咪酯组(对照组,40例)。采用共识睡眠日记和医院焦虑抑郁量表焦虑部分评估2组患者检查前1 d和检查后第1、3、7天的睡眠质量和焦虑状态。比较2组患者一般情况,麻醉时间,依托咪酯使用剂量,围术期不良反应和患者、内镜医师和麻醉医师满意率。结果 与对照组比较,观察组患者在检查后第1、3、7天的睡眠质量更高,医院焦虑抑郁量表焦虑部分评分更低(P＜0.05),依托咪酯使用量更少(P＜0.05),内镜医师和麻醉医师满意率更高(P＜0.05),低血压、恶心呕吐和低氧血症发生率更低(P＜0.05)。结论 与舒芬太尼比较,使用亚麻醉剂量艾司氯胺酮复合依托咪酯的老年女性在无痛胃镜检查后睡眠质量更好、焦虑情绪更少,且依托咪酯使用量更小、围术期不良反应发生率更低。 |

| Objective To explore the effect of subanesthetic doses of esketamine on sleep quality in elderly women after painless gastroscopy.Methods This study was a prospective research.Eighty elderly women who underwent gastroscopy under anesthesia at the First Affiliated Hospital of Anhui Medical University from July to September 2024 were randomly divided into two groups using a random number table:an observation group (n=40) receiving a combination of subanesthetic doses of esketamine and etomidate,and a control group (n=40) receiving sufentanil and etomidate.The sleep quality and anxiety status of the two groups of patients were evaluated using the Consensus Sleep Diary and The Hospital Anxiety and Depression Scale Anxiety Scale on 1 day before inspection and 1,3,and 7 days after inspection.Compare the general situation,anesthesia related time,dosage of etomidate,perioperative adverse reactions,and satisfaction rates of patients,endoscopists,and anesthesiologists between two groups of patients.Results Compared to the control group,patients in the observation group had higher sleep quality,lower The Hospital Anxiety and Depression Scale Anxiety Scale on days 1,3,and 7 after examination (P<0.05),lower etomidate dose (P<0.05) and higher satisfaction rates with anesthesia quality (P<0.05).The incidence of perioperative hypotension,nausea,vomiting,and hypoxemia also were lower compared to the control group (P<0.05).Conclusions Compared with sufentanil,a combination of anesthetically administered doses of ketamine and etomidate have better sleep quality and less anxiety after painless gastroscopy,as well as lower use of etomidate and lower incidence of perioperative adverse reactions among elderly women. |
