      目的 分析老年住院患者营养不良状况及影响因素,探讨营养不良与衰弱之间的相关性。方法回顾性分析 2020年 1月至2021 年 1 月在中国科学技术大学附属第一医院(安徽省立医院)老年科住院的 283例老年患者病历资料。入选患者均由专业人员进行老年综合信息评估,并分别使用营养评估量表(MNA)和衰弱量表(FRAIL)评估患者的营养和衰弱状况,统计老年住院患者营养不良发生情况;采用多因素 logistic 回归分析老年住院患者发生营养不良的风险因素;采用Spearman 秩相关分析老年住院患者衰弱与营养不良的相关性。结果 283例老年患者中,97例(34.3%)存在营养不良,186例(65.7%)营养状况正常或存在营养不良风险;68例(24.0%)衰弱,215例(76.0%)无衰弱或处于衰弱前期。多因素 logistic回归分析结果显示,共病个数(OR=1.408,95%CI:1.052~1.883)、糖尿病(OR=2.858,95%CI:1.414~5.774)、衰弱(OR=5.144,95%CI:2.414~10.962)、血红蛋白 (OR=0.973,95%CI:0.957~0.991)、白蛋白(OR=0.914,95%CI:0.848~0.985)、三酰甘油(OR=0.102,95%CI:0.047~0.220)是老年患者发生营养不良的影响因素(P<0.05)。老年住院患者MNA评分与FRAIL评分呈负相关。结论 共病个数、糖尿病、衰弱状态、血红蛋白、白蛋白、三酰甘油为老年住院患者营养不良的影响因素,应加强临床上对于营养不良的识别与管理;老年住院患者营养不良与衰弱显著相关。

      Objective To analyse the malnutrition status and influencing factors of elderly hospitalized patients,and explore the correlation between malnutrition and frailty.Methods The medical records of 283 elderly patients admitted to the department of geriatrics at the First Affiliated Hospital of the University of Science and Technology of China (Anhui Provincial Hospital) from January 2020 to January 2021 were analysed retrospectively.The enrolled patients were assessed by professional staff for comprehensive geriatric information,and the nutritional and debilitating conditions of the patients were assessed using the nutritional assessment scale (MNA) and the debilitation scale (FRAIL),respectively.Statistics on the occurrence of malnutrition in elderly inpatients;multifactorial logistic regression analysis was used to explore the risk factors for malnutrition in elderly inpatients;Spearman′s rank correlation analysis was used to explore the correlation between frailty and malnutrition in elderly inpatients.Results Among 283 elderly patients,97 (34.3%) were malnourished,186 (65.7%) were at risk of malnutrition and had normal nutritional status,68 (24.0%) were debilitated,and 215 (76.0%) were predebilitated and debilitated.The results of multifactorial logistic regression analysis showed that the number of co-morbidities (OR=1.408,95%CI:1.052-1.883),diabetes mellitus(OR=2.858,95%CI:1.414-5.774),debilitation(OR=5.144,95%CI:2.414-10.962),haemoglobin(OR=0.973,95%CI:0.957-0.991),albumin(OR=0.914,95%CI:0.848-0.985),and triglycerides(OR=0.102,95%CI:0.047-0.220) were the risk factors for the development of malnutrition in elderly patients (P<0.05).MNA score and FRAIL score were correlated and negatively correlated in elderly inpatients.Conclusions The number of co-morbidities,diabetes mellitus,debilitated state,haemoglobin,albumin,and triglycerides are influencing factors for malnutrition in elderly inpatients,and the identification and management of malnutrition should be strengthened in clinical practice.Malnutrition and debilitation in elderly inpatients is significantly correlated.
