目的 探讨基于交叉迁移理论的双下肢弹力带抗阻训练在脑卒中偏瘫患者康复中的应用效果。方法 前瞻性研究。选取2022年10月至2023年12月入住上海市第三康复医院康复科的脑卒中偏瘫患者84例,采用随机数字表法分为对照组及观察组,每组各42例。2组均接受常规治疗及康复护理,对照组实施患侧下肢弹力带抗阻训练,观察组实施双下肢弹力带抗阻训练。干预4周后比较2组患者下肢运动功能、平衡能力、步行能力、日常生活活动能力以及下肢肌肉表面肌电信号。结果 干预后,观察组患者的Fugl-Meyer运动功能评定量表下肢部分评分、Berg平衡量表评分、10 m最大步行速度及Barthel指数评分显著优于对照组(均P<0.05);观察组患者双下肢主要肌群的平均肌电值和均方根值显著优于对照组(均P<0.05)。结论 基于交叉迁移理论的双下肢弹力带抗阻训练能更有效地促进脑卒中偏瘫患者患侧下肢肌力的恢复,进一步提高运动功能、平衡能力、步行能力,改善日常生活活动能力。 |

Objective To investigate the effect of double lower extremity elastic bands resistance training based on cross-migration theory in the rehabilitation of hemiplegic patients with stroke.Methods Eighty-four cases of stroke hemiplegia patients who were admitted to the rehabilitation department of Third Rehabilitation Hospital of Shanghai City from October 2022 to December 2023 were selected and divided into the control group and the observation group by using the method of randomized numerical table,with 42 cases in each group.Both groups received conventional treatment and rehabilitation care,the control group implemented resistance training with elastic bands on the affected lower limbs,and the observation group implemented resistance training with elastic bands on both lower limbs.Lower extremity motor function,balance,walking ability,activities of daily living,and lower extremity muscle surface electromyographic signals were compared between the 2 groups after 4 weeks of intervention.Results After the intervention,the Fugl-Meyer Motor Function Rating Scale Lower Extremity Component Score,Berg Balance Scale Score,10-meter Maximum Walking Speed,and Barthel Index Score of the patients in the observation group were significantly better than those of the control group (all P<0.05);and the average EMG values and root mean square values of the major muscle groups of the lower extremities of the patients in the observation group were significantly better than those of the control group (all P<0.05).Conclusions Resistance training with double lower extremity elastic bands based on cross-migration theory can effectively promote the recovery of lower extremity muscle strength on the affected side of hemiplegic patients with stroke,further improve motor function,balance,walking ability,and improve the ability of activities of daily living. |
