目的 探讨高血压对老年期抑郁障碍患者不同睡眠阶段的睡眠功率谱密度及睡眠效率的影响。方法 通过抽样法选取合肥市第四人民医院老年心理科2019年1月至2024年6月期间收治的老年抑郁障碍患者。通过访谈收集参与者的社会人口学特征,并采用汉密尔顿抑郁评定量表来评估抑郁症状的严重程度。采用多导睡眠仪收集中老年抑郁症患者的睡眠信息,通过数据分析获得不同睡眠阶段不同频段的功率谱密度睡眠参数。根据是否合并高血压分组进行独立样本t检验。对睡眠数据和社会人口统计学特征进行斯皮尔曼相关分析,并通过逐步回归分析,建立多元回归方程分析各影响因素的影响程度。结果 纳入55例老年抑郁障碍患者,根据是否合并高血压分为2组:高血压组24例,非高血压组31例。2组患者的睡眠效率(t=3.506,P<0.001)、觉醒时间(t=-3.204,P=0.002)、N3期δ波功率谱密度(t=3.045,P=0.003)差异均有统计学意义。相关性分析结果显示,年龄与N1期δ波功率谱密度呈正相关(r=0.455,P=0.025),与N2睡眠时间呈负相关(r=-0.667,P<0.001);病程与N1时间(r=0.417,P=0.043)呈正相关,与N2%(r=-0.478,P=0.018)呈负相关。睡眠时长、睡眠潜伏期时长、是否有高血压对抑郁障碍患者的睡眠效率有影响(R2=0.350,F=9.176,P<0.001)。结论 合并高血压的老年抑郁障碍患者与不合并高血压的老年抑郁障碍患者睡眠特征存在明显差异。合并高血压的老年抑郁障碍患者深度睡眠质量及睡眠效率更差。 |

Objective To explore the impact of hypertension on the sleep of elderly patients with depressive disorders.Methods Select elderly patients with depression admitted to the department of geriatric psychology of the Fourth People′s Hospital of Hefei from January 2019 to June 2024 through sampling method.Socio-demographic characteristics of the participants were collected through interviews and the Hamilton Depression Rating Scale was used to assess the severity of depressive symptoms.Polysomnography was used to collect sleep information from middle-aged and elderly depressed patients,and data were analyzed to obtain power spectral density sleep parameters in different frequency bands of other sleep stages.Independent samples t-tests were performed according to whether co-morbid hypertension subgroups were present.Spearman′s correlation analysis was performed for sleep data and sociodemographic characteristics.The degree of influence of the influencing factors was also analyzed by stepwise regression analysis,and multiple regression equations were created to analyze the degree of influence of the influencing factors.Results Fifty-five elderly patients with depressive disorders were included in this study,and the elderly patients with depressive disorders who were divided into two groups according to the presence or absence of hypertensive disease.Hypertensive group(n=24)and non-hypertensive group(n=31) showed a significant difference in sleep efficiency(t=3.506,P<0.001),awakening time(t=-3.204,P=0.002),and N3 phase δ-wave power spectral density(t=3.045,P=0.003) were significantly different.The results of correlation analysis showed that age was significantly positively correlated with N1 δ-wave power spectral density(r=0.455,P=0.025) and negatively correlated with N2 sleep time(r=-0.667,P<0.001).Disease duration was significantly positively correlated with N1 time(r=0.417,P=0.043) and significantly negatively correlated with N2%(r=-0.478,P=0.018).Sleep duration,sleep latency duration,and the presence of hypertension affected sleep efficiency in patients with depressive disorders (R2=0.350,F=9.176,P<0.001).Conclusions There were significant differences in the sleep characteristics of elderly patients with depressive disorders with or without co-morbid hypertension.Elderly patients with depressive disorders with co-morbid hypertension has worse quality of deep sleep and sleep efficiency. |
