目的 评估三维能谱数字放射成像(3D-SDR)在早期慢性阻塞性肺疾病(COPD)患者中应用的可行性和临床应用价值,为COPD早期筛查提供新的技术。方法 采用前瞻性、自身对照的单中心设计。纳入2020年6月至2023年5月上海市第六人民医院就诊的198例早期COPD患者。所有受试者接受高分辨率CT(HRCT)和3D-SDR检查,影像结果由3位资深的胸肺放射科医师独立盲法评估。结果 在HRCT阴性的136例患者中,3D-SDR阴性率为97.1%(132/136);在HRCT阳性的62例患者中,3D-SDR阳性率为80.6%(50/62)。Pearson χ2检验显示2种影像学方法显著相关(χ2=129.64,r=0.809,P<0.001),Cohen kappa系数为0.805(P<0.001),提示诊断一致性较高。3D-SDR与HRCT的诊断早期COPD检出率分别为27.3%(54/198)和31.3%(62/198),提示两者均适用于早期筛查,HRCT的敏感性更优。结论 3D-SDR作为新的影像学检测手段,在COPD早期筛查中具有一定的临床价值,可作为COPD高风险人群的初步筛查方法。 |

Objective To evaluate the feasibility and clinical application value of three-dimensional spectral digital radiography (3D-SDR) in patients with early-stage chronic obstructive pulmonary disease (COPD),and to provide a new technique for early COPD screening.Methods This study adopted a prospective,self-controlled,single-center design and enrolled 198 early-stage COPD patients who visited Shanghai Sixth People′s Hospital between June 2020 and May 2023.All participants underwent both high-resolution computed tomography (HRCT) and 3D-SDR examinations.The imaging results were independently and blindly evaluated by three senior thoracic radiologists.Results Among 136 patients with HRCT negative results,the 3D-SDR negative rate was 97.1% (132/136);among the 62 patients with HRCT positivity,the 3D-SDR positivity rate was 80.6% (50/62).The Pearson chi square test showed a significant correlation between the two imaging methods (χ2=129.64, r=0.809, P<0.001),with a Cohen kappa coefficient of 0.805 (P<0.001),indicating high diagnostic consistency.The detection rates of early COPD by 3D-SDR and HRCT were 27.3% (54/198) and 31.3% (62/198),respectively, indicating that both are suitable for early screening and HRCT has better sensitivity.Conclusions As a novel imaging detection method,3D-SDR exhibits certain clinical value in the early screening of COPD and can serve as a preliminary screening tool for high-risk COPD populations. |
