| 目的 探讨新风胶囊治疗膝骨关节炎(KOA)的效果及对外周血NOD样受体蛋白3(NLRP3)炎症小体的影响。方法 采用前瞻性的研究方法,纳入2023年2月至2025年3月在安徽中医药大学第一附属医院住院的KOA患者60例。其中男13例,女47例；年龄44～68(57.2±7.0)岁；病程1～30(10.4±6.9)年。按照随机数字表法分为对照组和研究组,各30例。对照组患者接受硫酸氨基葡萄糖口服治疗,剂量:0.5 g/次,3次/d。研究组在对照组基础上加用新风胶囊,均治疗4周。观察2组患者治疗前后的临床疗效、中医证候积分、奎森功能演算指数(Lequesne评分)、疼痛视觉模拟评分法(VAS)评分、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分、健康评估问卷(HAQ)评分和36项简短健康调查表(SF-36)评分；检测血红细胞沉降率(ESR)、C反应蛋白(CRP)水平；用qRT-PCR法检测患者NLRP3表达水平；ELISA法检测血白细胞介素(IL)-1β、IL-6、IL-18、肿瘤坏死因子(TNF)-α、基质金属蛋白酶(MMP)-3、MMP-9表达水平。结果 2组患者治疗后症状均有所改善,运用新风胶囊治疗的KOA患者临床疗效优于对照组(P＜0.05)。与对照组比较,研究组患者治疗后中医证候积分、Lequesne评分、WOMAC评分、VAS评分和HAQ评分更低,SF-36评分更高(P＜0.05),血ESR、CRP、NLRP3、IL-1β、IL-6、IL-18以及MMP-3、MMP-9更低(P＜0.05)。结论 新风胶囊治疗KOA临床效果良好,可有效降低炎症反应,改善患者生活质量。 |

| Objective To explore the therapeutic effect of Xinfeng capsule on knee osteoarthritis (KOA) and its impact on the peripheral blood NOD like receptor protein 3 (NLRP3) inflammasome.Methods Using a prospective research method,60 patients with knee osteoarthritis admitted to the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine from February 2023 to March 2025 were included,including 13 males and 47 females;age range from 44 to 68 (57.2 ± 7.0) years old;the course of the disease ranges from 1 to 30 (10.4 ± 6.9) years.According to the random number table method,they were divided into a control group and a study group,with 30 cases in each study group.The control group patients received oral treatment with sulfated glucosamine at a single dose of 0.5 g,3 times a day.The study group received additional treatment with fresh air capsules on the basis of the control group for 4 weeks.The clinical efficacy,TCM syndrome score,Lequesne osteoarthritis index score,VAS score,WOMAC score,HAQ score,and SF-36 score of two groups of patients before and after treatment were observed;biochemical methods was used to detect erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) expression levels;Detect the expression level of NLRP3 in patients using qRT PCR method;ELISA was used to detect the expression levels of interleukin (IL)-1β,IL-6,IL-18,tumor necrosis factor (TNF)-α,matrix metalloproteinase-3 (MMP-3),and MMP-9.Results The symptoms of the two groups were improved after treatment,and the clinical efficacy of KOA patients treated with traditional Chinese patent medicines and simple preparations Xinfeng Capsule was better than that of the control group (P<0.05).Compared with the control group,the study group had lower TCM syndrome scores,Lequesne,WOMAC,VAS,and HAQ scores,and higher SF-36 scores after treatment (P<0.05).Compared with the control group,the study group had lower levels of ESR,CRP,NLRP3,IL-1β,IL-6,IL-18,as well as MMP-3 and MMP-9 expression after treatment (P<0.05).Conclusions The clinical application of Xinfeng capsules in the treatment of knee osteoarthritis is good,which can effectively reduce inflammatory reactions,improve patients′ quality of life,and enhance clinical efficacy. |
