| 目的 调查北京城市社区老年人口腔衰弱现状并分析其与体力衰弱的相关性,寻找共同的风险因素和干预目标。方法 横断面研究。于2022年3月至2023年6月在北京3家老年社区医疗中心进行。选用Fried衰弱表型量表评估体力衰弱,选用口腔衰弱指数评估口腔衰弱。采用多因素logistic回归法分析口腔衰弱的影响因素。结果 共纳入427例参与者,年龄(81.8±9.5)岁,其中口腔衰弱258例(60.42%)。每年到牙科诊所就诊次数少于1次(72.37%)和牙齿缺失(63.70%)是口腔衰弱老年人中最常见的条目。调整协变量后,logistic回归分析显示,体力衰弱(OR=1.44,95%CI:1.15~1.79,P=0.001)与口腔衰弱发生风险呈正相关。在体力衰弱的各条目中,疲乏(OR=2.35,95%CI:1.30~4.28,P=0.005)和步速缓慢(OR=1.80,95%CI:1.10~2.93,P=0.020)与口腔衰弱呈独立正相关。结论 体力衰弱的老年人应更加关注口腔健康问题,尤其是对疲乏和步速缓慢的老年人应注意对口腔衰弱的筛查。 |

| Objective To investigate the prevalence of oral frailty among the elderly in urban communities of Beijing and to analyze its relationship with physical physical frailty,identifying common risk factors and potential intervention targets.Methods This cross-sectional study was conducted from March 2022 to June 2023 in three elderly community medical centers in Beijing.The Fried Phenotype is used to assess physical frailty,while the Oral Frailty Index was employed to evaluate oral frailty.Multivariate logistic regression analysis was performed to identify factors influencing oral frailty.Results A total of 427 participants were included,with an average age of (81.8±9.5) years.Among them,258 individuals (60.42%) exhibited oral frailty.The most common issues among those with oral frailty were visiting a dental clinic less than once a year (72.37%) and tooth loss (63.70%).After adjusting for covariates,logistic regression analysis showed that physical frailty (OR=1.44,95%CI:1.15-1.79,P=0.001) is positively associated with the risk of oral frailty.Within the items of physical frailty,fatigue(OR=2.35,95%CI:1.30-4.28,P=0.005)and slow walking speed(OR=1.80,95%CI:1.10-2.93,P=0.020)are independently positively associated with oral frailty.Conclusions Individuals with physical frailty should pay greater attention to oral health issues.Fatigue and slow walking speed may serve as sensitive indicators of oral frailty. |
