      目的 探讨血清脑源性神经营养因子(BDNF)和尿酸(UA)水平与代谢综合征(MetS)患者失眠的关系及其诊断价值。方法 回顾性选取2023年10月至2024年7月蚌埠医科大学第一附属医院收治的100例MetS合并失眠患者作为观察组(合并组),41例MetS患者为对照组(代谢组);根据失眠严重程度将合并组分为轻度组(59例)、中度组(27例)、重度组(14例);另选取同期于该院体检健康者73例作为健康对照组。统计分析各组BDNF、UA水平及两者对合并失眠患者的诊断价值。结果 合并组BDNF水平显著低于健康组和代谢组(P<0.01),合并组UA水平高于健康组(P<0.01);重度组中血清BDNF水平、UA水平低于中度组和轻度组(P<0.01);多因素logistic回归分析显示,BDNF是MetS合并失眠的保护因素,UA为危险因素;ROC曲线显示,BDNF诊断MetS合并失眠的AUC为0.721(95%CI:0.644~0.798)、UA诊断AUC为0.674(95%CI:0.593~0.755)、BDNF联合UA诊断AUC为0.789(95%CI:0.722~0.856)。结论 BDNF、UA水平与MetS合并失眠有一定相关性,两者联合诊断MetS合并失眠具有较好的临床应用价值。

      Objective To explore the relationship between serum brain-derived neurotrophic factor (BDNF) and uric acid (UA) levels and insomnia in patients with metabolic syndrome (MetS) and their diagnostic value analysis.Methods A retrospective study was conducted in 100 patients with Metabolic Syndrome (MetS) complicated with insomnia,who were admitted to the First Affiliated Hospital of Bengbu Medical University from October 2023 to July 2024,serving as the observation group (combined group),and the combined group was divided into a mild group (59 cases),a moderate group (27 cases),and a severe group (14 cases) according to the severity of insomnia,and 73 cases of healthy people with physical examination were selected as healthy controls during the same period in the hospital.In addition,73 cases were selected as the healthy control group.The levels of BDNF and UA in each group were statistically analyzed and the diagnostic value of BDNF and UA in the combined group was analyzed.Results The BDNF level in the combined group was significantly lower than that in the healthy and metabolic groups (P<0.01),and the UA in the combined group was significantly higher than those in the healthy group (P<0.01);the serum BDNF level and UA level in the severe group were significantly lower than those in the moderate and mild groups (P<0.01);regression analysis showed that BDNF was a protective factor against insomnia in the combination of MetS,and UA was a risk factor;ROC curves showed that the AUC for BDNF diagnosis of MetS combined with insomnia was 0.721 (95%CI:0.644-0.798),and that for UA diagnosis was 0.674 (95% CI:0.593-0.755),and the AUC for BDNF combined with UA diagnosis was 0.789 (95% CI:0.722-0.856).Conclusions The levels of BDNF and UA have a certain correlation with insomnia combined with MetS,and the combination of the two has a good reference value for diagnosing insomnia combined with MetS.
