| 强直性脊柱炎(AS)是一种以中轴关节慢性炎症为核心特征的自身免疫性疾病,临床主要表现为持续性腰背疼痛,常伴外周关节及多系统受累。在疾病进展严重的情况下,患者可能出现脊柱的强直或畸形,对其身体健康及生活质量造成显著影响。AS发病机制复杂,涉及遗传因素、免疫反应及肠道微生物群等多个方面,具体机制尚未完全阐明。目前认为病理过程主要涉及炎症反应、骨质破坏及异位骨化3个方面,且免疫炎症被认为是AS病理进程中的核心环节。因此,控制免疫炎症的发生与发展是治疗AS的关键所在。中药具有多靶点、多途径、多成分等药理学作用,可通过抗炎抗免疫防治AS,且临床效果显著,但聚焦免疫炎症与中医药干预罕见相关文献报道。因此,本文综述了AS的免疫炎症反应及中医药从抗炎抗免疫角度治疗AS的近年研究报道,旨在为中医药防治AS进一步研究提供参考,指明新的思路与方法。 |

| Ankylosing Spondylitis (AS) is an inflammatory autoimmune disease characterized clinically by chronic lower back pain,primarily affecting the axial joints but may also involve peripheral joints and various other organs.In severe cases of the disease progression,patients may develop spinal stiffness or deformity,significantly impacting their quality of life and physical health.The etiology of AS is complex,associated with genetics,immunity,gut microbiota,among others,with its pathogenesis remaining unclear.Among these,inflammatory response,bone destruction,and ectopic ossification are the three main pathological changes in AS.Immune inflammation is the core stage of the pathological process of AS,and controlling immune inflammation is key to treatment.Traditional Chinese medicine (TCM),with its pharmacological actions of targeting multiple points,pathways,and components,can alleviate AS through anti-inflammatory effects,showing significant clinical outcomes.However,literature focusing on immune inflammation and TCM intervention is scarce.Therefore,this paper reviews the latest research reports on the immune inflammatory response of AS and the treatment of AS from the anti-inflammatory and anti-immune perspectives by traditional Chinese medicine,aiming to provide references and new ideas and methods for further research for the prevention and treatment of AS with traditional Chinese medicine. |
