| 随着我国人口老龄化进程的加速,传统以肢体功能为核心的康复模式已难以满足“多病共存、多器官功能衰退”的老年人群需求。多项研究证实,以评估-运动-营养-心理-药物-睡眠“六维协同”为特征的老年脏器功能康复可显著改善虚弱、心衰、糖尿病、慢性肾脏病等老年常见慢病及老年综合征的预后,降低再住院率并提高生活质量。本文系统阐述老年脏器康复的概念、循证证据及规范化路径,提出了“评估-干预-教育”闭环流程,构建Ⅰ-Ⅱ-Ⅲ期序贯康复路径,为老年人提供系统、专业、全程康复医疗服务。 |

| With the acceleration of China′s population aging process,the traditional rehabilitation model centered on limb function has become difficult to meet the needs of elderly populations with "coexistence of multiple diseases and decline in multiple organ functions".Multiple studies have confirmed that elderly visceral rehabilitation,characterized by the "six-dimensional synergy" of assessment-exercise-nutrition-psychology-medication-sleep,can significantly improve the prognosis of common chronic diseases and geriatric syndromes in the elderly,such as frailty,heart failure,diabetes,and chronic kidney disease,reduce readmission rates,and enhance quality of life.This article systematically expounds the concept,evidence-based evidence,and standardized pathway of elderly visceral rehabilitation,proposes a closed-loop process of "assessment-intervention-education",and constructs a sequential rehabilitation pathway of stages Ⅰ-Ⅱ-Ⅲ,providing systematic,professional,and comprehensive rehabilitation medical services for the elderly. |
