| 目的 探讨围手术期多样化护理干预对肾癌患者手术后疼痛管理和情绪影响的效果。方法 回顾性选取2023年12月至2024年12月海军军医大学第一附属医院(上海长海医院)收治的行肾癌根治术和肾部分切除术的120例肾癌患者病历资料,按照随机数字表法分为干预组和对照组各60例。对照组实施围手术期常规护理,干预组实施围手术期全程护理干预。记录2组围手术期患者的心理状态剖面图(POMS)紧张-焦虑分量表评分,术前术后第1天的心率、血压、呼吸频率以及视觉模拟评分法(VAS)评估的患者疼痛程度,患者的住院时间。结果 在手术后,干预组患者的VAS评分[4.9(4.7,5.1)分]明显低于对照组[5.5(5.2,5.7)分],P<0.01；干预组患者的住院时间[(3.02±0.37)d]显著少于对照组[(3.36±0.35)d],P<0.05。术前和术后干预组患者的紧张感和焦虑感评分均低于对照组(P＜0.001)；术前和术后干预组患者的心率、呼吸频率和平均动脉压均低于对照组。结论 围手术期全程护理干预方案能够稳定术前、术后肾癌患者的情绪,改善术后疼痛程度以及缩短患者住院时间。 |

| Objective To explore the effects of perioperative diversified nursing interventions on postoperative pain management and emotional impact in patients with renal cell carcinoma.Methods We retrospectively selected the medical records of 120 patients with renal cell carcinoma who underwent radical nephrectomy or partial nephrectomy at the First Affiliated Hospital of Naval Medical University (Shanghai Changhai Hospital) from December 2023 to December 2024.These patients were divided into an intervention group and a control group,with 60 patients in each group,using a random number table method.The control group received routine perioperative care,while the intervention group received comprehensive perioperative nursing intervention.We recorded the Psychological Profile of Mood States (POMS) tension-anxiety subscale scores for both groups of patients during the perioperative period,as well as their heart rate,blood pressure,respiratory rate,and pain levels assessed using the Visual Analogue Scale (VAS) before surgery and at the first postoperative day.We also recorded the patients′ hospitalization time.Results After surgery,the VAS score of patients in the intervention group [4.9(4.7,5.1) points] was significantly lower than that of the control group [5.5(5.2,5.7) points],P<0.01;the hospitalization duration of patients in the intervention group [(3.02±0.37) d] was significantly shorter than that of the control group [(3.36±0.35) d],P<0.05.The scores for tension and anxiety of patients in the intervention group were lower than those of the control group both before and after surgery (P<0.001);the heart rate,respiratory rate,and mean arterial pressure of patients in the intervention group were lower than those of the control group both before and after surgery.Conclusions The perioperative comprehensive nursing intervention program can stabilize the emotions of patients with renal cancer before and after surgery,alleviate postoperative pain,and shorten their hospital stay. |
