| 随着全球人口老龄化进程的加速,老年群体的健康促进已成为健康老龄化的核心指标,日益受到学界关注。世界卫生组织提出,老年人健康包括躯体健康、心理健康、社会和道德健康。从中西医协同的角度,建立科学有效的老年人健康状态筛查与评定体系,对于维护老年人群健康状态、促进健康老龄化具有重要意义。目前,我国尚缺乏中西医协同的健康状态筛查与综合评定规范化共识。为此,浙江中医药大学附属第一医院(浙江省中医院)、中日友好医院牵头,联合中国老年保健协会智慧医养专业委员会、浙江省慢性病防治中西医结合指导中心、中国研究型医院学会专家团队,组织制订本标准。本标准基于老年人健康状态评估的最新指南、循证研究等文献资料,结合中西医老年医学及相关领域权威专家临床经验,针对各层级医疗机构医务人员,切实将老年人健康状态的西医评估和中医体质辨识相结合,构建科学、规范的健康状态筛查与综合评定体系,为早期识别健康状态受损,动态管理健康状态减退进程,维持功能独立,提升维护老年人健康状态的中西医协同诊疗能力,最终实现健康老龄化目标提供临床指导。 |

| With the acceleration of global population aging,the health promotion of the elderly has emerged as a core indicator of healthy aging and garnered growing attention in academia.The World Health Organization defines the health of the elderly as encompassing physical health,mental health,social health,and moral health.The establishing a scientific and effective screening and assessment system for the elderly′s health status from the perspective of synergy between Traditional Chinese Medicine (TCM) and Western Medicine is of great significance for maintaining the functional status of the elderly population and promoting healthy aging.Currently,there remains a paucity of standardized consensus on the screening and comprehensive assessment of health status based on TCM-Western medicine synergy in China.To address this gap,the First Affiliated Hospital of Zhejiang Chinese Medical University (Zhejiang Provincial Hospital of Chinese Medicine) and China-Japan Friendship Hospital took the lead,jointly with the Smart Healthcare and Elderly Care Professional Committee of the Chinese Aging Well Association,Zhejiang Provincial Guidance Center for Integrated Traditional Chinese and Western Medicine in Chronic Disease Prevention and Control,and the expert team of the Chinese Research Hospital Association,in organizing the formulation of this standard.Based on the latest guidelines,evidence-based studies,and other relevant literature on the elderly′s health status assessment,as well as clinical expertise from authoritative experts in the field,this standard is targeted at medical staff in medical institutions at all levels.It effectively integrates Western medicine assessment of the elderly′s health status with TCM constitution identification,constructing a scientific and standardized screening and comprehensive assessment system.This standard provides clinical guidance for early detection of health impairment,dynamic monitoring and management of health status decline,maintenance of functional independence,and enhancement of the synergetic diagnosis and treatment capacity of integrated TCM and Western medicine in safeguarding the elderly′s health,ultimately advancing the goal of healthy aging. |
