| 随着全球人口老龄化进程加速,衰弱作为一种常见的老年综合征,已成为影响老年人生活质量与健康预期的重要问题。衰弱不仅是评估老年人整体健康状况的敏感指标,较之时序年龄,其能更精准地预测失能、住院及死亡等不良健康结局。因此,衰弱状态应作为制订个体化治疗与照护方案的核心依据,早期开展衰弱筛查与多维度评估,并将其纳入老年人健康管理的重要环节。建议将衰弱筛查前移至社区和全科医疗,建立多学科协作机制,实施基于老年综合评估的个性化干预。通过全程化、分层化的衰弱管理,可有效延缓衰弱进展,提升老年人生活质量,推进健康老龄化。 |

| With the accelerating process of population aging globally,frailty,as a common geriatric syndrome,has become a critical issue affecting the quality of life and health expectancy of older adults.Frailty is not only a sensitive indicator for assessing the overall health status of older adults,but compared with chronological age,it more accurately predicts adverse health outcomes such as disability,hospitalization,and mortality.Therefore,the frailty status should serve as a core basis for developing individualized treatment and care plans.Early frailty screening and multidimensional assessment should be carried out,incorporating frailty management as an essential part of elderly health management.It is recommended that screening be shifted to community and primary care settings,with the establishment of a multidisciplinary collaboration mechanism to implement personalized interventions based on comprehensive geriatric assessments.Establishing a comprehensive and stratified frailty management model can effectively delay its progression,enhance the quality of life for older adults,and represents a critical pathway towards achieving the healthy aging strategy. |
