| 目的 探讨残余胆固醇(RC)与缺血性卒中患者早期认知功能障碍发生的相关性。方法 选取2022年7月至2024年7月在南京医科大学附属苏州医院神经内科就诊初发急性缺血性脑卒中患者300例。根据90 d随访结果,运用蒙特利尔认知评估量表(MoCA)分为卒中后认知功能障碍(PSCI)组117例,非PSCI组183例。对两组数据进行统计学分析,分析RC与初发急性缺血性脑卒中患者早期认知障碍发生相关性。应用受试者操作特征(ROC)曲线评估RC对PSCI的预测价值。结果 PSCI组年龄、糖尿病、总胆红素、间接胆红素、总胆固醇、低密度脂蛋白胆固醇、极低密度脂蛋白胆固醇、同型半胱氨酸、糖化血红蛋白、RC与非PSCI组比较,差异均有统计学意义(均P<0.05);logistic回归分析显示,年龄、间接胆红素、同型半胱氨酸、糖化血红蛋白、低密度脂蛋白胆固醇、RC与PSCI发生相关(均P<0.05);ROC曲线分析显示,RC的ROC曲线下面积为0.713(95%CI:0.653~0.773)。结论 RC与缺血性卒中早期认知障碍发生存在相关性。 |

| Objective To explore the correlation between remnant cholesterol (RC) and the occurrence of early cognitive dysfunction in patients with ischemic stroke.Methods A total of 300 patients with first-episode acute ischemic stroke admitted to the Department of Neurology,Suzhou Hospital Affiliated to Nanjing Medical University from July 2022 to July 2024 were enrolled.Based on 90-day follow-up results and Montreal Cognitive Assessment (MoCA) scores,they were divided into the poststroke cognitive impairment(PSCI)group (117 cases) and the non-PSCI group (183 cases).Statistical analyses were performed to explore the correlation between remnant cholesterol and early cognitive impairment in these patients.The receiver operator characteristic (ROC) curve was used to evaluate the predictive value of the RC for PSCI.Results Univariate analysis revealed significant differences between the PSCI group and the non-PSCI group in age,diabetes,total bilirubin,indirect bilirubin,total cholesterol,low-density lipoprotein,very low-density lipoprotein,homocysteine,glycated hemoglobin,and remnant cholesterol (P<0.05).Logistic regression analysis indicated that age,indirect bilirubin,homocysteine,glycated hemoglobin,low-density lipoprotein and remnant cholesterol were associated with PSCI(all P<0.05).ROC curve analysis showed that the area under the curve for RC was 0.713 (95%CI:0.653-0.773).Conclusions RC is associated with the occurrence of early cognitive impairment in patients with ischemic stroke. |
