      目的 探讨社区老年人群中胰岛素样生长因子结合蛋白(IGFBP)-2、IGFBP-3对肌少症的诊断价值。方法 采用病例对照设计,选取2024年6月新疆维吾尔自治区木垒县社区老年人流行病学调查中的人群,依据《中国肌肉减少症诊疗指南(2024版)》标准诊断的62例肌少症患者作为病例组,并纳入114名非肌少症者作为对照组。采用受试者工作特征(ROC)曲线评价IGFBP-2与IGFBP-3对肌少症的诊断价值。结果 与非肌少症组相比,肌少症组具有更高的收缩压、舒张压、高密度脂蛋白胆固醇、IGFBP-2,更低的腰围、体重指数、握力、6 m步速、四肢骨骼肌质量、胰岛素样生长因子(IGF)-1、IGFBP-3(P<0.05)。ROC曲线显示,IGFBP-2诊断肌少症、肌肉质量下降的曲线下面积(AUC)为0.906(灵敏度98.39%,特异度74.56%)、0.936(灵敏度98.59%,特异度80.95%),IGFBP-3诊断肌少症、肌肉质量下降的AUC为0.757(灵敏度75.81%,特异度66.67%)、0.772(灵敏度78.87%,特异度66.67%)。结论 IGFBP-2、IGFBP-3对肌少症、肌肉质量下降具有较好的诊断价值,IGFBP-2对肌少症、肌肉质量下降的诊断价值优于IGFBP-3。

      Objective To investigate the diagnostic value of insulin-like growth factor-binding protein (IGFBP)-2 and IGFBP-3 for sarcopenia in an elderly community population.Methods In a community-based epidemiological survey conducted among older adults in Mulei County, Xinjiang Uygur Autonomous Region, in June 2024,62 individuals diagnosed with sarcopenia and 114 without sarcopenia were recruited as cases and controls, respectively, according to the Chinese Guidelines for the Diagnosis and Treatment of Sarcopenia (2024 edition).Receiver operating characteristic (ROC) curve analysis was used to evaluate the diagnostic performance of IGFBP-2 and IGFBP-3 for sarcopenia.Results Compared to the control group,the sarcopenia group exhibited significantly higher systolic blood pressure,diastolic blood pressure,high-density lipoprotein,and IGFBP-2 levels,along with lower waist circumference,body mass index,grip strength,6-meter walking speed,appendicular skeletal muscle mass,IGF-1,and IGFBP-3 levels (all P<0.05).ROC analysis demonstrated that IGFBP-2 achieved an area under the curve (AUC) of 0.906 (sensitivity 98.39%,specificity 74.56%) for diagnosing sarcopenia and 0.936 (sensitivity 98.59%,specificity 80.95%) for detecting reduced muscle mass.For IGFBP-3,the AUC were 0.757 (sensitivity 75.81%,specificity 66.67%) for sarcopenia and 0.772 (sensitivity 78.87%,specificity 66.67%) for reduced muscle mass.Conclusions IGFBP-2 exhibits strong diagnostic potential for sarcopenia and muscle mass decline,providing a reference for early clinical identification.
