      为探索老年慢性病患者营养治疗由临床规范向生活实践转化的可行路径,构建契合我国老年慢性病人群特点的生活化营养方案,本文系统梳理了世界卫生组织、欧洲临床营养与代谢学会等权威机构最新指南,并结合国家卫生健康委员会2024年发布的《高血压等慢性病营养和运动指导原则》及中国老年人平衡膳食宝塔核心内容,综合分析了国内老年群体膳食习惯与慢性病流行现状。研究显示,通过“评估-干预-指导-随访”全流程路径,可实现国际营养共识本土化、临床规范生活化、药-膳协同个体化,切实改善老年慢性病患者营养治疗依从性,提升营养干预的实际应用成效。

      To explore the feasible path for the transformation of nutritional therapy for elderly patients with chronic diseases from clinical standards to life practice,and to construct a lifestyl-oriented nutritional program that fits the characteristics of elderly patients with chronic diseases in China,this article systematically reviews the latest guidelines from authoritative institutions such as the World Health Organization and the European Society for Clinical Nutrition and Metabolism.It also combines the Nutrition and Exercise Guiding Principles for Hypertension and Other Chronic Diseases issued by the National Health Commission in 2024 and the core content of the Balanced Diet Pagoda for Chinese Elderly,comprehensively analyzing the dietary habits of the domestic elderly population and the current epidemic status of chronic diseases.The research shows that through the whole process of "assessment-intervention-guidance-follow-up",it is possible to achieve the localization of international nutritional consensus,the application of clinical standards in daily life,and the individualization of medicine-diet coordination,effectively improving the compliance of elderly patients with chronic diseases in nutritional therapy and enhancing the practical application effectiveness of nutritional intervention.
