      目的 探讨达芬奇机器人手术系统辅助肾动脉瘤切除重建术的临床疗效。方法 2022年6月,陆军军医大学第一附属医院血管外科收治1例行达芬奇机器人辅助肾动脉瘤切除血管重建术患者,因检查发现右肾动脉近肾门处动脉瘤(2枚),且瘤体长径均>2 cm,在达芬奇机器人辅助下行肾动脉瘤切除血管重建术。采用门诊随访,了解患者肾动脉血流和靶器官情况。随访截至2022年7月。结果 成功行达芬奇机器人辅助肾动脉瘤切除血管重建术,手术经腹膜后途径完成,完整切除2枚长径约2.5 cm的肾动脉瘤,并采用端端吻合联合端侧吻合的复合技术完成肾动脉重建。手术总时间为328 min,术中出血量约500 mL。肾动脉吻合口吻合良好,肾动脉血流通畅。腹部戳卡孔共4个(1.2 cm戳卡2个、0.8 cm戳卡2个),术中及术后均无腹腔大出血、感染等并发症。术后住院时间10 d。随访1个月,生存良好。术后复查腹部超声提示右肾动脉吻合口血流通畅,右肾血流充盈好。结论 达芬奇机器人手术系统辅助肾动脉瘤切除+血管重建术安全、可行,随访1个月结果令人满意。

      Objective To explore the clinical efficacy of Da Vinci robotic surgical system-assisted renal artery aneurysm resection and vascular reconstruction.Methods In June 2022,a patient undergoing Da Vinci robot-assisted renal artery aneurysm resection and vascular reconstruction was admitted to the Vascular Surgery Department of the First Affiliated Hospital of the Military Medical University of the People′s Liberation Army.The patient was found to have two aneurysms near the hilum of the right renal artery,both with a length greater than 2 cm.The Da Vinci robot-assisted renal artery aneurysm resection and vascular reconstruction was performed.Outpatient follow-up was conducted to assess the renal artery blood flow and target organ status.The follow-up ended in July 2022. Results The patient successfully underwent Da Vinci robot-assisted renal artery aneurysm resection and vascular reconstruction.The surgery was performed via a retroperitoneal approach,with complete resection of two renal artery aneurysms approximately 2.5 cm in length,and renal artery reconstruction was achieved using a combination of end-to-end and end-to-side anastomosis techniques.The total operation time was 328 minutes,with an intraoperative blood loss of approximately 500 mL.The renal artery anastomosis was well performed,and the renal artery blood flow was unobstructed.There were a total of four abdominal trocar ports (two 1.2 cm and two 0.8 cm),and there were no complications such as major abdominal bleeding or infection during or after the surgery.The postoperative hospital stay was 10 days.The patient was followed up for one month and is currently doing well.Postoperative abdominal ultrasound examination indicated unobstructed blood flow at the renal artery anastomosis site,and good blood flow in the right kidney. Conclusions Da Vinci robotic surgical system-assisted renal artery aneurysm resection and vascular reconstruction is safe and feasible,and the one-month follow-up results are satisfactory.
