      目的 探讨菊粉补充干预方案对老年糖尿病合并肌少症患者血液学指标及肠道微生态的影响。方法 本研究采用随机、单盲、对照试验设计。选取2022年10月至2024年12月在首都医科大学宣武医院就诊的70例老年糖尿病合并肌少症患者,按照随机数字表法分为观察组[35例,常规降糖+营养+运动+菊粉(10 g/d)]和对照组(35例,常规降糖+营养+运动+安慰剂),在干预期间对两组患者血糖及降糖方案进行动态随访和调整,干预6个月后,比较两组干预前后糖脂代谢、握力、骨骼肌质量、体能状况、炎症指标、肠道菌群水平变化情况。结果 干预6个月后,观察组空腹血糖、胰岛素抵抗指数水平较对照组明显更低(均P<0.05),高密度脂蛋白胆固醇水平较对照组明显更高(P<0.05)。干预3个月后,两组患者的握力、四肢骨骼肌质量指数(ASMI)、简易体能状况量表(SPPB)得分比较,差异均无统计学意义(均P>0.05);干预6个月后,观察组患者握力、ASMI和SPPB得分较对照组更高(均P<0.05)。干预6个月后,观察组超敏C反应蛋白、肿瘤坏死因子-α、白细胞介素-6水平较对照组更低(均P<0.05),观察组厚壁菌门水平、变形菌门水平较对照组更低(均P<0.05),拟杆菌门水平、放线菌门水平较对照组更高(均P<0.05)。结论 在综合管理的基础上补充菊粉,有助于改善老年糖尿病合并肌少症患者的血糖控制、肌肉功能、炎症状态及肠道微生态。

      Objective To investigate the effect of inulin supplementation intervention on hematological indexes and intestinal microecology in elderly patients with diabetes mellitus complicated with sarcopenia.Methods This study adopted a randomized,single blind,controlled trial design.Seventy elderly patients with diabetes complicated with myopenia who visited Xuanwu Hospital of Capital Medical University from October 2022 to December 2024 were selected,They were divided into observation group (n=35,conventional hypoglycemic + nutrition + exercise + inulin 10 g/d intervention) and control group (n=35,conventional hypoglycemic + nutrition + exercise + placebo) according to the random number table method.During the intervention period,the blood glucose and hypoglycemic regimen of the two groups were dynamically followed up and adjusted.Both groups were intervened for 6 months.The changes of glucose and lipid metabolism,grip strength,skeletal muscle quality,physical condition,inflammatory index and intestinal flora level before and after intervention were compared between the two groups.Results After 6 months of intervention,the levels of fasting blood glucose and insulin resistance index in the observation group were significantly lower than those in the control group (both P<0.05),and the level of high density lipoprotein was significantly higher than that in the control group (P<0.05).There was no significant difference in grip strength,ASMI and SPPB scores between the two groups after 3 months of intervention (all P>0.05).After 6 months of intervention,the grip strength,ASMI and SPPB scores of the observation group were significantly higher than those of the control group (all P<0.05).After 6 months of intervention,the levels of high-sensitivity C-reactive protein,tumor necrosis factor-α and interleukin-6 in the observation group were significantly lower than those in the control group (all P<0.05).The levels of Firmicutes and Proteobacteria in the observation group were significantly lower than those in the control group (both P<0.05),and the levels of Bacteroidetes and Actinobacteria were significantly higher than those in the control group (both P<0.05).Conclusions Supplementation of inulin on the basis of comprehensive management is helpful to improve blood glucose control,muscle function,inflammatory state and intestinal microecology in elderly diabetic patients with sarcopenia.
