      目的 探讨2型糖尿病(T2DM)患者糖脂代谢与糖尿病视网膜病变(DR)之间的相关性。方法 回顾性分析2022年1月至2023年2月在北部战区总医院内分泌科收治的597例2型糖尿病患者的临床资料。根据眼底检查结果分为2组:非糖尿病视网膜病变组(NDR组,375例)和糖尿病视网膜病变组(DR组,222例)。收集患者基线数据,计算糖化血红蛋白(HbA1c)与高密度脂蛋白胆固醇(HDL-C)的比率,并分析其与DR的相关性。结果 DR组HbA1c/HDL-C高于NDR组,差异有统计学意义(P<0.001)。Mantel-Haenszel的卡方趋势分析表明HbA1c/HDL-C与DR之间存在显著的线性趋势(P<0.001),HbA1c/HDL-C与DR呈正相关,r=0.237,P<0.001。二元logistic回归分析显示,收缩压、糖尿病病程、HbA1c/HDL-C是T2DM患者DR的独立危险因素。限制性立方样条分析显示,HbA1c/HDL-C与DR之间存在非线性关系(P总趋势=0.012 1,P非线性=0.020)。受试者操作特征曲线分析显示,HbA1c/HDL-C对DR的诊断准确性最高,受试者操作特征曲线下面积为0.660,灵敏度为42.9%,特异度为78.8%。结论 高HbA1c/HDL-C是2型糖尿病患者DR的独立危险因素,其对DR具有良好的预测价值。

      Objective To explore the correlation between glucose and lipid metabolism and diabetic retinopathy (DR) in patients with type 2 diabetes mellitus (T2DM).Methods A retrospective analysis was conducted on the clinical data of 597 patients with type 2 diabetes admitted to the Endocrinology Department of the General Hospital of the Northern Theater Command of the Chinese People′s Liberation Army from January 2022 to February 2023.Based on the results of fundus examination,the patients were divided into two groups:the non-diabetic retinopathy group (NDR group,375 cases) and the diabetic retinopathy group (DR group,222 cases).Baseline data of the patients were collected,and the ratio of glycated hemoglobin (HbA1c) to high-density lipoprotein cholesterol (HDL-C) was calculated,and its correlation with DR was analyzed.Results HbA1c/HDL-C ratio in DR group was higher than that in NDR group (P<0.001).Mantel-Haenszel′s chi-square trend analysis indicated a notable linear trend (P<0.001) between HbA1c/HDL-C ratio and DR.HbA1c/HDL-C revealed positive correlations with DR,r=0.237,P<0.001.Binary logistic regression analysis showed systolic blood pressure,diabetes course,fasting blood glucose and HbA1c/HDL-C ratio as independent risk factors for DR in T2DM patients.Restrictive cubic spline analysis demonstrated a significant nonlinear relationship between HbA1c/HDL-C and DR (Ptotal trend=0.012 1,Pnonlinear=0.020).ROC curve analysis identified that HbA1c/HDL-C had the highest diagnostic accuracy for DR,with an area under the ROC curve of 0.660,42.9% sensitivity,and 78.8% specificity.Conclusions Inceased HbA1c/HDL-C is an independent risk factor for DR in patients with type 2 diabetes,which also has good predictive value for DR.
