| 目的 探讨低温等离子消融术治疗顽固性带状疱疹后遗神经痛(PHN)的效果。方法 采用随机对照试验。选取2020年3月至2024年3月于宁夏医科大学总医院接受治疗的220例顽固性PHN患者,经抽签法将患者随机分为两组,对照组(110例)口服普瑞巴林治疗,治疗组(110例)口服普瑞巴林联合低温等离子消融术治疗。比较两组患者疼痛程度、睡眠质量、生活质量、炎症因子等方面的差异并评估其临床疗效,观察其不良反应。结果 治疗组有效率为95.45%,明显优于对照组的83.64%(χ2=8.206,P=0.004)；治疗后,两组患者的视觉模拟评分量表得分、匹兹堡睡眠质量指数量表得分、皮肤病生活质量指数、白细胞介素-1β水平、白细胞介素-6水平、白细胞介素-8水平比治疗前显著降低,差异均有统计学意义(均P<0.05);治疗组治疗后效果优于对照组,差异均有统计学意义(均P<0.001)；对照组患者嗜睡、头晕发生率高于治疗组(P＜0.001)。结论 低温等离子消融术治疗顽固性PHN能减少炎症因子的产生,减轻患者疼痛,改善患者的睡眠质量。 |

| Objective To explore the efficacy of low-temperature plasma ablation in the treatment of intractable postherpetic neuralgia (PHN).Methods A randomized controlled trial was employed.A total of 220 patients with intractable PHN treated at the General Hospital of Ningxia Medical University from March 2020 to March 2024 were selected.Patients were randomly divided into two groups by lot drawing.The control group (110 cases) received oral pregabalin treatment,while the treatment group (110 cases) received oral pregabalin combined with low-temperature plasma ablation.The differences in pain severity,sleep quality,quality of life,inflammatory factors,and clinical efficacy were compared between the two groups,and adverse reactions were observed.Results The effective rate in the treatment group was 95.45%,significantly better than that in the control group (83.64%) (χ2=8.206,P=0.004).After treatment,the visual analog scale,Pittsburgh Sleep Quality Index,Dermatology Life Quality Index,levels of interleukin-1β,interleukin-6,and interleukin-8 in both groups were significantly lower than before treatment,with statistically significant differences (all P<0.05).The post-treatment effect in the treatment group was superior to that in the control group,with statistically significant differences (all P<0.001).The incidence of drowsiness and dizziness was higher in the control group than in the treatment group (P<0.001).Conclusions Low-temperature plasma ablation can reduce the production of inflammatory factors,alleviate pain,and improve sleep quality in patients with intractable PHN. |
