| 目的 分析B超联合促甲状腺素(TSH)与甲状腺球蛋白(Tg)比值检查诊断甲状腺结节良恶性的应用价值,旨在为临床诊断提供参考依据,以提高该病诊断的准确率。方法 筛选出60例甲状腺结节患者作为研究对象,所有患者均手术病理诊断证实,其中良性结节43例,恶性结节17例；对所有患者进行B超检测及TSH/Tg检查,以手术病理诊断结果为金标准,评价B超检测、TSH/Tg检查以及B超联合TSH/Tg检查诊断甲状腺结节良恶性的应用价值,包括准确度、敏感度、特异度、阳性预测值、阴性预测值。结果 B超检查检出甲状腺结节良性33例,恶性11例；TSH/Tg检查检出甲状腺结节良性30例,恶性9例,B超联合TSH/Tg检查检出甲状腺结节良性39例,恶性15例。B超检查诊断甲状腺结节良恶性的准确度、敏感度、特异度、阳性预测值、阴性预测值依次为73.3%、84.6%、52.4%、76.7%、64.7%；TSH/Tg检查依次为65.0%、78.9%、40.9%、69.8%、52.9%；B超联合TSH/Tg检查依次为90.0%、95.1%、78.9%、90.7%、88.2%；B超联合TSH/Tg检查诊断的准确度、敏感度、特异度、阳性预测值、阴性预测值均高于B超检查与TSH/Tg检查,各组差别明显。结论 B超联合TSH/Tg检查可提高甲状腺结节良恶性的鉴别价值。 |

| Objective To analyze the value of Bmode ultrasound combined with TSH/Tg in the diagnosis of benign and malignant thyroid nodules,and to provide reference for clinical diagnosis. Methods Sixty patients with thyroid nodules were selected from June 2017 to June 2018 in our hospital.All patients were confirmed by surgical pathology,including 43 benign nodules and 17 malignant nodules.Preoperative examinations included B-mode ultrasound and TSH/Tg in all patients.To pathology results for the gold standard.The application value of B-ultrasound,TSH/Tg and B-ultrasound combined with TSH/Tg in the diagnosis of benign and malignant thyroid nodules were evaluated,including accuracy,sensitivity,specificity,positive predictive value and negative predictive value. Results Thirty-three cases of benign thyroid nodules and 11 cases of malignant thyroid nodules were detected by B-ultrasound.Thirty cases of benign thyroid nodules and 9 cases of malignant thyroid nodules were detected by TSH/Tg.Thirty-nine cases of benign thyroid nodules and 15 cases of malignant thyroid nodules were detected by B-ultrasound combined with TSH/Tg.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of B-ultrasound in the diagnosis of benign and malignant thyroid nodules were 73.3%,84.6%,52.4%,76.7%,and 64.7%,respectively.The results of TSH/Tg were 65.0%,78.9%,40.9%,69.8%,52.9%.The results of B-ultrasound combined with TSH/Tg were 90.0%,95.1%,78.9%,90.7%,and 88.2%,respectively.The accuracy,sensitivity,specificity,positive predictive value,and negative predictive value of B-ultrasound combined with TSH/Tg were higher than those of B-ultrasound and TSH/Tg.Each difference is obvious. Conclusion B-mode ultrasound combined with TSH/Tg has high value in the diagnosis of benign and malignant thyroid nodules. |
