      目的 探讨前列地尔联合α-硫辛酸对糖尿病周围神经病变患者神经传导速度及氧化应激的影响。方法 采用随机数字法将152例糖尿病周围神经病变患者分为两组,每组76例。两组均行糖尿病基础治疗,对照组加用前列地尔注射液10 μg/d,观察组在对照组的基础上给予α-硫辛酸注射液600 mg/d,两组均连续治疗2周。比较两组治疗前后血糖水平[空腹血糖(FBG)、餐后2 h血糖(2hPG)及糖化血红蛋白(HbA1c)]、正中神经与腓总神经传导速度[感觉神经传导速度(SNCV)及运动神经传导速度(MNCV)]、神经功能症状评分[多伦多临床神经病变评分系统(TCSS)]、疼痛评分[视觉模拟评分(VAS)]、氧化应激[硝基酪氨酸(NT)、超氧化物歧化酶(SOD)、丙二醛(MDA)]的变化及临床疗效。结果 治疗后,观察组总有效率(96.05%)显著高于对照组(76.32%),P<0.05。治疗后,两组FBG、2hPBG及HbA1c水平均显著降低(P<0.05),且观察组明显低于对照组(P<0.05)。治疗后,两组正中神经、腓总神经的SNCV、MNCV水平均明显升高(P<0.05),且观察组显著高于对照组(P<0.05)。治疗后,两组TCSS、VAS评分均显著降低(P<0.05),且观察组明显低于对照组(P<0.05)。治疗后,两组NT、MDA水平均明显降低(P<0.05),且观察组显著低于对照组(P<0.05);两组SOD水平均明显升高(P<0.05),且观察组显著高于对照组(P<0.05)。结论 前列地尔联合α-硫辛酸治疗糖尿病周围神经病变效果显著,可明显改善患者神经功能,提高周围神经传导速度,减弱氧化应激损伤。

      Objective To investigate the effects of alprostadil combined with α-lipoic acid on patients with diabetic peripheral neuropathy and its effect on nerve conduction velocity and oxidative stress.Methods A total of 152 patients with diabetic peripheral neuropathy were randomly divided into two groups,with 76 cases in each group.All patients were treated with the basic treatment of diabetes.The control group was given alprostadil injection(10 μg/d),and the observation group was given α-lipoic acid injection(600 mg/d).Both groups were treated continuously for two weeks.The change and clinical efficacy before and after treatment,blood glucose levels (FBG,2hPBG,HbA1c),median nerve and common peroneal nerve conduction velocity (SNCV,MNCV),neurological symptom score (TCSS),pain score (VAS),oxidative stress (NT,SOD,MDA) were compared between the two groups.Results After treatment,the total efficiencies of the observation group and the control group were 96.05% and 76.32% respectively.The total efficiency of the observation group was significantly higher than that of the control group (P<0.05).After treatment,the levels of FBG,2hPBG and HbA1c in the two groups were significantly decreased (P<0.05),and the observation group was significantly lower than the control group (P<0.05).After treatment,the SNCV and MNCV levels of the median nerve and common peroneal nerve were significantly increased in the two groups (P<0.05),and which in observation group were significantly higher than those of the control group (P<0.05).After treatment,the TCSS and VAS scores of the two groups were significantly decreased (P<0.05),and which in observation group was significantly lower than those of the control group (P<0.05).After treatment,the levels of NT and MDA in the two groups were significantly decreased (P<0.05),and the observation group was significantly lower than the control group (P<0.05).The SOD levels in the two groups were significantly increased (P<0.05),and the observation group was significantly higher than the control group (P<0.05).Conclusion The effect of alprostadil and α-lipoic acid on diabetic peripheral neuropathy is significant,which can significantly improve neurological function,increase peripheral nerve conduction velocity,and reduce oxidative stress of patients.
