| 目的 探讨影响高龄脑卒中患者静脉留置针留置时间的影响因素及护理对策。方法 选取解放军总医院第二医学中心神经内科在2019年6月至2020年2月收治的实施留置针输液治疗的高龄脑卒中患者84例作为研究对象,进行回顾性分析,观察留置针留置时间,探讨其影响因素并提出护理对策。结果 患者静脉留置针留置时间为(7.77±1.87)d；出血性脑卒中患者留置时间长于缺血性脑卒中患者[(8.20±1.89) d,(7.28±1.75) d,P<0.05]；患者是否卧床的留置时间差异无统计学意义(P>0.05)；穿刺部位位于前臂静脉者留置时间长于穿刺部位位于手背静脉者[(8.24±1.80) d,(7.33±1.85) d,P<0.05]；未输入高渗性液体患者留置时间长于输入高渗性液体患者[(8.41±1.70) d,(7.16±1.89) d,P<0.05]；操作者为高年资护士留置时间长于操作者为低年资护士[(8.20±1.95) d, (7.39±1.73) d,P<0.05]。结论 高龄脑卒中患者留置针留置时间长短与患者脑卒中类型、穿刺部位、是否输入高渗性液体、护士年资密切相关,可采取针对性护理措施,延长留置时间,减少患者痛苦。 |

| Objective To explore the influence factors and nursing countermeasures of Closed IV Catheter System retention time in the elderly stroke patients.Methods We retrospectively collected clinical data of 84 elderly stroke patients admitted to Neurology ward,the Second medical center of PLA General Hospital from June 2019 to February 2020 and treated with indwelling needle infusion treatment to explore the influence factors and nursing countermeasures of Closed IV Catheter System retention time.Results The mean Closed IV Catheter System retention time was(7.77±1.87)d.The retention time of the patients with hemorrhagic stroke was significantly longer than that of the patients with ischemic stroke[(8.20±1.89) d vs (7.28±1.75) d,P<0.05].There was no significant difference in the patients in bed or not (P>0.05).The retention time of forearm veinpuncture was significantly longer than that of hand dorsal veinpuncture[(8.24±1.80) d vs (7.33±1.85) d,P<0.05].The retention time of isotonic fluid infusion was significantly longer than that of hypertonic fluid infusion[(8.41±1.70) d vs (7.16±1.89) d,P<0.05].The retention time of senior nurses was significantly longer than that of junior nurses[(8.20±1.95) d vs (7.39±1.73) d,P<0.05].Conclusion The Closed IV Catheter System retention time of the elderly stroke patients is closely related to stroke types,veinpuncture sites,kinds of transfusion liquids and the nurse qualification.We can take the corresponding nursing measures to prolong the retention time and alleviate the sufferings of patients. |
