      目的 探讨雄激素受体(AR)在乳腺癌组织中的表达情况,以及AR的表达与患者复发转移的关系。方法 选取2015年10月至2019年8月就诊于北京医院乳腺外科及肿瘤内科患者共235例,应用免疫组织化学技术检测癌组织中AR的表达,并采用χ2检验比较患者年龄、分子分型、肿瘤大小、淋巴结转移情况、组织学分级及雌激素受体、孕激素受体、Ki67、人类表皮生长因子受体2(HER2)的表达情况与AR表达的关系;根据患者肿瘤复发转移的情况,通过χ2检验、单因素及多因素分析,分析患者无病生存期(DFS)与AR的表达是否相关。结果 AR在77.9%的乳腺癌组织中有阳性表达,雌激素受体阳性的患者,AR阳性率高达84%(P<0.05)。年龄≥65岁的患者AR阳性表达率更高88.1%(P<0.05),在三阴性乳腺癌患者中AR阳性表达率更低(51.7%,P<0.05)。中位随访时间35个月(2~120月),共有72例患者出现复发转移,复发转移患者AR阳性率低于未复发的患者(P<0.05),AR阴性的患者有更易复发的趋势(P=0.072)。AR阳性和阴性患者的DFS分别为67月与46月,差异无统计学意义(P=0.070)。结论 AR在77.9%的乳腺癌组织中有阳性表达,AR阴性的乳腺癌患者可能更易出现复发转移。

      Objective To detect the androgen receptor (AR) expression in breast cancer tissues and the relationship between AR expression and patients′ recurrence and metastasis.Methods A total of 235 cases of breast surgery and oncology patients in Beijing hospital were selected from October 2015 to August 2019,Immunohistochemistry was used to detect the expression of AR in cancer tissues,and χ2 test was used to compare the relationship of AR expression in patients with age,molecular subtype,tumor size,lymph node metastasis,histological grade,the expression of estrogen receptor,progesterone receptor,Ki67,human epidermal growth factor receptor 2(HER2).According to the recurrence and metastasis of tumor,χ2 test,univariate and multivariate analysis were conducted to analyze the difference in AR expression between patients with recurrence and without recurrence.Results AR was positively expressed in 77.9% of breast cancer tissues,and AR positive was rate up to 84% in patients with hormone receptor positive (P<0.05).The positive expression rate of AR was 88.1% (P< 0.05) higher in patients with age ≥65,and 51.7% (P<0.05) lower in triple negative breast cancer patients.The median follow-up time was 35 months (2-120 m).A total of 72 patients had recurrence and metastasis,and the positive rate of AR in patients with recurrence and metastasis was lower than that in patients without recurrence (P<0.05).Patients with negative AR tended to be more likely to relapse (P= 0.072).The disease-free survival (DFS) of AR positive and negative patients were 67 months vs. 46 months,respectively.There was no statistically significant difference (P=0.070).Conclusion AR is positively expressed in 77.9% of breast cancer tissues.Breast cancer patients with negative AR may be more prone to recurrence and metastasis.
