      糖尿病是造成老年衰弱/肌少症的重要原因,老年衰弱是生活自理向失能发展的过渡期,也是为延缓失能调整干预措施的最后窗口。胰岛素分为内源性和外源性,内源性胰岛素是机体内唯一降低血糖的激素,外源性胰岛素主要用于治疗糖尿病。2022年适逢胰岛素临床使用100周年,在过去的100多年里,科学家们对胰岛素有了越来越深入的认识,新型胰岛素层出不穷,使糖尿病患者的医疗照护和健康长寿有了长足进步。该文通过问答的方式从以下方面解析胰岛素对糖尿病衰弱/肌少症进展的双向作用:(1)胰岛素分泌与健康增龄的关系;(2)老年期胰岛素不足或胰岛素抵抗对肌少症、衰弱的危害;(3)老年糖尿病对衰弱/肌少症的危害及防控要点;(4)老年糖尿病合并衰弱的管理目标;(5)胰岛素治疗对衰弱/肌少症的影响。

      Diabetes is an important contributor to frailty/sarcopenia.Frailty is the transition period from self-maintenance,which is also viewed as the last opportunity to adjust interventions to delay disability.Insulin is classified as endogenous and exogenous.Endogenous insulin is the only hormone in the body that lowers blood sugar,while exogenous insulin is mainly used to treat diabetes.2022 marks the 100th anniversary of the clinical application of insulin.Over the past 100 years,insulin is gained with deeper comprehension,as emerging new types of insulin make great strides in medical care and healthy longevity of diabetic population.This article analyzes the bidirectional effect of insulin on the progression of frailty sarcopenia with diabetes from the following aspects through questions and answers:(1) the relationship between insulin secretion and healthy aging;(2) the hazard of insulin insufficiency or insulin resistance to sarcopenia and frailty;(3) the hazard of elderly diabetes to frailty/sarcopenia and the key points of prevention and control;(4) management goals of elderly diabetes with frailty;(5) the effect of insulin therapy on frailty/sarcopenia.
