| 目的 比较环泊酚与丙泊酚对髋关节置换手术患者围术期炎症指标及对术后恢复情况的影响。方法 回顾性收集在2022年5—10月于中国科学技术大学附属第一医院(安徽省立医院)行髋关节置换术的252例患者临床资料。根据患者麻醉维持用药情况,分为环泊酚组及丙泊酚组。回顾患者麻醉用药、围术期检验结果、围术期临床数据等资料,分析环泊酚与丙泊酚对患者的恢复[如气管拔管时间、麻醉复苏室(PACU)停留时间、术后住院日等]及炎症指标有无影响。基于混合线性模型比较2组患者围术期中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)、淋巴细胞/单核细胞比值(LMR)及全身炎症标志物C反应蛋白(CRP)及红细胞沉降率(ESR)的变化。结果 2组患者年龄、性别、美国麻醉医师协会分级、疾病构成比及慢性病史等基线情况比较,差异无统计学意义(P>0.05)。2组患者手术时间、术后住院日、气管拔管时间、PACU停留时间等比较,差异无统计学意义(P>0.05)。单因素混合线性模型表明,2组患者围术期NLR、PLR、LMR及CRP、ESR变化差异无统计学意义(P>0.05),经年龄、性别、高血压、糖尿病变量校正后结果相同。结论 在髋关节置换手术中,相较于丙泊酚,应用环泊酚进行麻醉维持是安全、有效的,2种药物对于全身炎症反应及术后恢复的影响差异无统计学意义。 |

| Objective To evaluate the effects of ciprofol and propofol on perioperative inflammatory markers and postoperative recovery in patients with hip replacement surgery.Methods Clinical data of 252 patients with hip replacement surgery in the First Affiliated Hospital of University of Science and Technology of China(Anhui Provincial Hospital)from May to October 2022 were retrospectively collected.Patients were categorized into two groups:ciprofol group and propofol group according to the status of anesthesia maintenance drugs.Anesthesia medication, perioperative blood test results,perioperative clinical data were reviewed to analyze the effects on recovery (such as extubation time,PACU stay time, postoperative hospital stay) and inflammatory markers of patients.Linear mixed regression models were used to compare the changes in perioperative NLR,PLR,LMR,systemic inflammatory marker CRP and ESR between ciprofol group and propofol group.Results There were no significant differences in age,sex,ASA grading,disease composition or chronic history between the two groups (P>0.05).There were no significant differences in operation time,extubation time,postoperative hospital stay or PACU stay time (P>0.05).The univariable mixed linear model showed that there were no significant changes in NLR,PLR,LMR, CRP or ESR between the two groups (P>0.05),the results were the same after adjusting variables for age,sex,hypertension,diabetes.Conclusions Compared with propofol,ciprofol is safe and effective for the Anesthesia maintenance of patients undergoing hip replacement.There is no significant difference between the two drugs on systemic inflammatory response and postoperative recovery. |
