      目的 分析血管生成素样蛋白4(ANGPTL4)、微小RNA-124(miR-124)在老年急性脑出血并发癫痫患者中的表达及其临床意义。方法 选取2020年1月至2022年1月首都医科大学附属北京地坛医院顺义院区收治的72例急性脑出血并发癫痫患者为急性脑出血并发癫痫组,另选取同期收治的单纯急性脑出血患者50例为急性脑出血组,健康体检的健康志愿者50例为对照组。检测各组血清ANGPTL4水平、miR-124 mRNA表达量。结果 与对照组比较,急性脑出血组、急性脑出血并发癫痫组ANGPTL4水平较高,miR-124 mRNA表达量较低(P<0.05);与急性脑出血组比较,急性脑出血并发癫痫组ANGPTL4水平较高,miR-124 mRNA表达量较低(P<0.05)。ANGPTL4与癫痫严重程度、认知障碍、脑卒中史呈正相关,miR-124与癫痫严重程度、认知障碍、脑卒中史呈负相关(P<0.05)。多因素logistic回归分析,结果显示认知障碍、脑卒中史、ANGPTL4、miR-124为影响老年急性脑出血并发癫痫的危险因素。ROC曲线显示,ANGPTL4、miR-124联合检测对老年急性脑出血并发癫痫的预测价值高于单项检测预测(P<0.05)。结论 老年急性脑出血并发癫痫患者ANGPTL4表达较高,miR-124表达较低;ANGPTL4、miR-124与患者癫痫严重程度、认知障碍、脑卒中史相关,且两项联合检测对老年急性脑出血并发癫痫的预测价值较高。

      Objective To analyze the expression and correlation of serum angiopoietin-like protein 4 (ANGPTL4) and microRNA-124 (Minor RNA-124,miR-124) in elderly patients with acute cerebral hemorrhage complicated by epilepsy.Methods A total of 72 patients with acute cerebral hemorrhage complicated by epilepsy who were treated in our hospital from January 2020 to January 2022 were selected as the acute cerebral hemorrhage complicated with epilepsy group,and 50 patients with simple acute cerebral hemorrhage who were treated in the same period were selected as the acute cerebral hemorrhage group.50 healthy volunteers were the control group.The levels of serum ANGPTL4 and miR-124 mRNA expression in each group were detected.Results Compared with the control group,the acute cerebral hemorrhage group and the acute cerebral hemorrhage complicated with epilepsy group had higher levels of ANGPTL4 and lower miR-124 mRNA expression (P<0.05).The higher the level,the lower the miR-124 mRNA expression (P<0.05).ANGPTL4 was positively correlated with epilepsy severity,cognitive impairment,and history of stroke,while miR-124 was negatively correlated with epilepsy severity,cognitive impairment,and history of stroke (P<0.05).As shown in Table 4,a logistic regression model was established with epilepsy severity,cognitive impairment,stroke history,ANGPTL4 and miR-124 as independent variables,taking whether epilepsy occurred in elderly acute cerebral hemorrhage as the dependent variable.Logistic Multivariate regression analysis,showed that cognitive impairment,stroke history,ANGPTL4 and miR-124 were risk factors affecting acute cerebral hemorrhage and epilepsy in the elderly.The ROC curve showed that the predictive value of the two combination in elderly patients with acute cerebral hemorrhage complicated with epilepsy was higher than that of ANGPTL4 and miR-124 alone (P<0.05).Conclusions The expression of ANGPTL4 in elderly patients with acute cerebral hemorrhage complicated by epilepsy is higher,and the expression of serum miR-124 is lower.Serum ANGPTL4 and miR-124 are related to the severity of epilepsy,cognitive impairment,and history of stroke.has a higher predictive value.
