| 目的 了解老年患病群体对医养结合的个性化、多层次需求,探讨构建城市老年患者分层医养结合体系。方法 基于Delphi法设计本研究调查指标,选取合肥市6家医养结合机构的城市老年患者作为调查对象,完成相应问卷409例。结果 职业、可支配月收入、医疗保险、对医养结合的认知、失能程度是医养结合机构照护意愿的可能影响因素。对于不同职业、可支配月收入、对医养结合认知水平、失能程度及医保类型的老年患者需进行针对性的分层医养。结论 城市老年患者,应根据老年患者病情和实际需求做出相应选择。基于老年综合评估,以城市基层医养结合机构为基础,三甲医院开设的医养结合模式老年科,可作为医护专业人才培训基地,以保障医养结合机构服务质量,从而构建行之有效的城市分层医养结合服务体系。 |

| Objective To comprehend the personalized and multi-level needs of elderly patients for the integration of medical and nursing care,and explore the construction of a hierarchical medical and nursing care integration system for urban elderly patients.Methods Based on the Delphi method,this study designed survey indicators and selected urban elderly patients from 6 medical and elderly care institutions in Hefei as the survey subjects,completing 409 corresponding questionnaires.Results Occupation,disposable monthly income,medical insurance,awareness of the integration of medical and elderly care,and degree of disability are potential influencing factors for the willingness of medical and elderly care institutions.Targeted hierarchical medical care is required for elderly patients with different occupation,disposable monthly income,cognitive level of medical and elderly care,disability level,and medical insurance type.Conclusions Urban elderly patients should make corresponding choices based on their condition and actual needs.Based on the comprehensive evaluation of the elderly,and based on urban grassroots medical and elderly care integration institutions,the geriatric department of the medical and elderly care integration model set up by tertiary hospitals can serve as a training base for medical and nursing professionals to ensure the service quality of medical and elderly care integration institutions,thus constructing an effective urban layered medical and elderly care integration service system. |
