| 目的 探讨肠内营养方案对肌肉减少性吞咽困难患者生活质量的影响。方法 回顾性分析2019年1月到2021年12月就诊于浙江中医药大学附属第二医院老年医学科参加老年综合评估患者的病历资料,筛查了符合纳入标准的60例患者。按照是否使用肠内营养制剂分为营养液治疗组(治疗组,30例),常规饮食组(对照组,30例),所有纳入患者均进行了健康进食指导,治疗组在健康进食指导的基础上加用肠内营养乳剂。在治疗4、8、12周后比较2组洼田饮水试验、C反应蛋白、血清白蛋白、血红蛋白、简易营养评估量表(MNA)评分、Barthel 指数、体重、体重指数指标的改善情况,并进行治疗前后对比。结果 经过12周健康进食指导后,对照组和治疗组治疗前后的Barthel指数、简易营养评估量表评分、洼田饮水试验结果、C反应蛋白水平较前好转(P＜0.05),差异有统计学意义。治疗组洼田饮水试验、C反应蛋白在治疗4、8、12周后低于对照组(P＜0.05),血清白蛋白、血红蛋白、Barthel指数在治疗8、12周后高于对照组(P<0.05),MNA评分在治疗12周后高于对照组(P＜0.05)。结论 对于肌肉减少性吞咽困难患者早期补充肠内营养制剂可在短期内快速改善营养状况,减轻吞咽困难的症状,提高生活质量。 |

| Objective To explore the effects of enteral nutrition emulsion on the quality of life of elderly patients with sarcopenic dysphagia.Methods A total of 60 elderly patients with sarcopenic dysphagia who were attended in the hospital from January 2019 to December 2021 were enrolled.They were divided into control group (n=30) and treatment group (n=30) based on whether enteral nutrition emulsion was used.The control group was given healthy eating instruction.The treatment group was given enteral nutrition emulsion on the basis of healthy eating instructions.Kubota drinking test,C-reactive protein,serum albumin,hemoglobin,Barthel index,MNA score,body weight and BMI were compared between the two groups.Results After 12 weeks of healthy eating guidance,the Barthel index,MNA score,Kubota drinking test,and C-reactive protein all improved in the two groups (P<0.05).Kubota drinking test,C-reactive protein in treatment group were better than those in control group (P<0.05) after 4 weeks and between 12 weeks.The serum albumin,hemoglobin,Barthel index in treatment group were higher than those in control group (P<0.05) after 8 weeks and between 12 weeks.The MNA score were higher than those in control group (P<0.05) after 12 weeks.Conclusions Guidance on healthy eating can improve the quality of life of the elderly patients with sarcoepnic dysphagia.Early supplementation of enteral nutrition emulsion treatments can rapidly improve nutritional status,relieve symptoms of dysphagia,and improve quality of life in a short period of time. |
