      目的 调查分析吸烟与男性2型糖尿病(T2DM)患者糖尿病肾脏病(DKD)及其组成部分之间的关系。方法 收集2022年1月至2023年1月在安徽医科大学附属省立医院内分泌科住院的男性2型糖尿病患者534例,根据尿白蛋白/肌酐比(UACR)或估算的肾小球滤过率(eGFR)结果,分为单纯T2DM组、T2DM合并DKD组。对2组吸烟状态和吸烟数量进行调查和分组分析,在调整年龄、糖尿病病程、空腹血糖、糖化血红蛋白、体重指数、收缩压、舒张压、血脂等因素后,采用多变量logistic回归模型评估吸烟与尿白蛋白-肌酐比和eGFR的关系。结果 现在吸烟者的DKD调整优势比显著高于非吸烟者(OR=1.66,95%CI:1.03~2.68),且随着吸烟指数的增加,大量白蛋白尿和低eGFR的发生率呈增加的趋势(P值均<0.05)。结论 在男性2型糖尿病患者中,吸烟与DKD发生有关,且存在一定的剂量-反应关系。

      Objective To investigate and analyze the relationship between smoking and diabetic kidney discase (DKD) and its components in male patients with type 2 diabetes (T2DM).Methods Five hundred and thirty-four male patients with type 2 diabetes who were hospitalized in the department of endocrinology,Provincial Hospital Affiliated to Anhui Medical University from January,2022 to January,2023 were collected.According to the results of urinary albumin creatinine ratio (UACR) or estimated glomerular filtration rate (eGFR),they were divided into T2DM alone group,T2DM combined with DKD group.The smoking status and smoking number of the two groups were investigated and grouped.After adjusting for age,duration of diabetes,fasting blood glucose,glycosylated hemoglobin,body mass index,systolic blood pressure,diastolic blood pressure,blood lipids and other factors,multivariate logistic regression model was used to evaluate the relationship between smoking and urinary albumin creatinine ratio and eGFR.Results Current smokers had a considerably increased odds ratio of DKD compared to non-smokers(OR=1.66,95%CI:1.03-2.68).Additionally,there was a tendency for the prevalence of macroalbuminuria and low eGFR to rise with a rising smoking index (P<0.05 for all).Conclusions In male patients with T2DM,smoking is associated with the occurrence of DKD,and there is a dose-response relationship in some extent.
