      目的 通过研究幽门螺杆菌(Hp)感染与动脉粥样硬化性疾病(AS)相关性,分析其危险因素,为临床治疗提供一些理论依据。方法 将2018年8月至2021年7月期间来联勤保障部队第九〇九医院(厦门大学附属东南医院)健康医学科进行体检的12 560名体检者作为研究对象,从中抽取体检者72名被确诊为冠状动脉粥样硬化 (AS组)、48名被确诊为血栓形成性脑卒中(血栓性脑卒中组),58名被确诊为脑梗死(脑梗死组),同时抽取100名健康体检者作为对照组,然后对上述体检者分别进行13C尿素呼气试验,检测Hp感染情况,所有受试者空腹抽血检测同型半胱氨酸(Hcy)、尿酸(UA)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)、三酰甘油(TG),分析其与Hp感染的关系。结果 AS组Hp检出率56.94%(41/72);脑梗死组Hp检出率60.34%(35/58);血栓性脑卒中组Hp检出率58.33%(28/48);对照组Hp检出率27.0%(27/100)。AS组、脑梗死组、血栓性脑卒中组TC、TG、LDL-C的检查结果均高于对照组,HDL-C低于对照组,上述指标差异均有统计学意义(P<0.05)。比较AS组、脑梗死组、血栓性脑卒中组的TC、TG、LDL-C,结果阳性患者均高于Hp阴性患者,但是HDL-C低于Hp阴性患者,差异有统计学意义(P<0.05)。与Hp阴性组相比,Hp阳性组的UA、LDL-C、TG、Hcy及年龄均较高,而HDL-C较低,差异有统计学意义(P<0.01)。多因素logistic回归分析提示,HDL-C是抗Hp感染的保护因子(P<0.01)。结论 Hp与AS之间是具有一定相关性的。Hp感染可显著损害脂质代谢,高HDL-C水平可减少Hp感染,可作为血管保护因子。

      Objective To study the correlation between Helicobacter pylori (Hp) infection and atherosclerotic disease (AS),analyze its risk factors,and provide some theoretical basis for clinical treatment.Methods From August 2018 to July 2021,12 560 physical examiners came to Department of Health Medicine of the 909th Hospital of the Joint Logistics Support Force (Dongnan Hospital of Xiamen University) for physical examination.In this study,physical examiners were selected AS research objects.Among the selected physical examiners,72 were diagnosed with coronary atherosclerosis (AS group) and 48 with thrombotic stroke (thrombotic stroke group).58 patients were diagnosed with cerebral infarction (cerebral infarction group),and 100 healthy customers were selected as the control group.13C urea breath test was performed on the above-mentioned patients to detect Hp infection.Homocysteine (Hcy),uric acid (UA),high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterol (LDL-C),total cholesterol (TC) and triacylglycerol (TG) were detected by fasting blood sampling in all subjects,and their relationship with Hp infection was analyzed.Results The detection rate of Hp in AS group was 56.94%(41/72).The detection rate of Hp in cerebral infarction group was 60.34%(35/58).The detection rate of Hp in thrombotic stroke group was 58.33%(28/48).The detection rate of Hp in control group was 27.0%(27/100).The levels of TC,TG and LDL-C in AS group,cerebral infarction group and thrombotic stroke group were all higher than those in control group,and only HDL-C was lower than that in control group,the above indexes were significantly different,and the differences were statistically significant (P<0.05).TC,TG and LDL-C in AS group,cerebral infarction group and thrombotic stroke group were compared.The results showed that positive patients were higher than HP-negative patients,but HDL-C was lower than HP-negative patients,and the difference was statistically significant (P<0.05).Compared with the HP-negative group,the UA,LDL-C,TG,Hcy and age in the HP-positive group were higher than those in the negative group,while the HDL-C was lower than that in the negative group,with statistical significance (P<0.01).Logistic multivariate analysis suggested that HDL-C was a protective factor against Hp infection (P<0.01).Conclusions There is a certain correlation between infection and AS,Hp infection can significantly impair lipid metabolism,and high HDL cholesterol levels can reduce Hp infection and act as a vascular protective factor.
