| 目的 探索D-二聚体、癌胚抗原(CEA)和糖链抗原19-9(CA199)在胃癌患者肿瘤负荷和疗效评价中的预测价值。方法 选取128例胃癌患者作为研究对象,按照肿瘤负荷程度,分为无肿瘤负荷组(56例)和有肿瘤负荷组(72例),测定患者血浆D-二聚体、CEA和CA199水平,比较两组之间及有肿瘤负荷组化疗前后三项指标水平的差异,分析三项指标对胃癌患者肿瘤负荷和化疗疗效的预测效能。结果 D-二聚体、CEA和CA199对胃癌患者肿瘤负荷预测的灵敏度分别为63.9%、58.3%、30.6%,D-二聚体和CEA联合预测的灵敏度为77.8%；D-二聚体、CEA和CA199对晚期胃癌患者化疗后进展预测的灵敏度分别为87.0%、65.2%、30.4%,D-二聚体和CEA联合预测的灵敏度为95.7%。结论 多项指标联合的预测效能要优于单项指标。 |

| Objective To explore the predictive value of D-dimer,CEA and CA199 in tumor load and therapeutic evaluation of gastric cancer patients.Methods The subjects who were 128 gastric cancer patients were divided into non-tumor load group (56 cases) and tumor load group (72 cases) according to the degree of tumor load.And then,by measuring and comparing the plasma levels of D-dimer,CEA and CA199 between groups,to analyze the predictive efficiency of three indicators for cancer load in patients with gastric cancer.Meanwhile,the plasma levels of three indicators before and after chemotherapy in tumor load group was analyzed by measuring and comparing,the predictive efficiency of three indicators for chemotherapy efficacy in patients with advanced gastric cancer.Results The sensitivity of D-dimer,CEA and CA199 for predicting cancer load in patients with gastric cancer was 63.9%,58.3%,30.6%,respectively.The sensitivity of combined prediction of D-dimer and CEA was 77.8%.The sensitivity of D-dimer,CEA and CA199 to predict the progression of advanced gastric cancer patients after chemotherapy was 87.0%,65.2%,30.4%,respectively.The sensitivity of combined prediction of D-dimer and CEA was 95.7%.Conclusion The combined predictive efficiency of multi indicator combination is better than that of single indicator. |
