| 目的 探讨原发性高血压患者超敏C反应蛋白(hs-CRP)与臂-踝动脉脉搏波速度(ba-PWV)的关系。方法 按照入选及排除标准从参加体检人群中选取591例高血压患者纳入研究。根据hs-CRP水平,将受检者分成hs-CRP＜1 mg/L组、hs-CRP＜3 mg/L组、hs-CRP≥3 mg/L组,比较上述各组ba-PWV水平及异常率。根据ba-PWV水平,将受检者分成ba-PWV正常组(ba-PWV＜1 400 cm/s)和ba-PWV异常组(ba-PWV≥1 400 cm/s),比较两组hs-CRP水平。对ba-PWV影响因素进行多因素logistic回归分析。结果 hs-CRP＜1 mg/L组、hs-CRP＜3 mg/L组和hs-CRP≥3 mg/L组ba-PWV分别为(1 592.9±268.5)cm/s、(1 659.1±292.1)cm/s、(1 649.5±314.7)cm/s,三组差异无统计学意义(F=2.896,P=0.056)。上述各hs-CRP水平组ba-PWV异常率分别为72.0%、81.2%、81.3%,χ2=6.821,P=0.033；hs-CRP＜3mg/L组和hs-CRP≥3mg/L组均显著高于hs-CRP＜1 mg/L组,P均＜0.017。ba-PWV正常组和异常组ln(hs-CRP)水平分别为(-0.26±1.00)mg/L、(-0.06±1.04)mg/L,P均＜0.05。logistic回归分析结果显示:hs-CRP不是ba-PWV的独立影响因素,年龄、收缩压、三酰甘油、空腹血糖及糖化血红蛋白是ba-PWV的独立影响因素。结论 影响ba-PWV的因素有年龄、收缩压、三酰甘油及血糖水平,hs-CRP并非ba-PWV的独立影响因素。 |

| Objective To explore the relationship between high-sensitivity C-reactive protein(CRP) and brachial-ankle pulse wave velocity in patients with essential hypertension.Methods According to the inclusion and exclusion criteria,591 patients with hypertension were enrolled in the study.The subjects were divided into three groups according to the hs-CRP:＜1 mg/L group,＜3 mg/L group and≥3 mg/L group.The Ba-PWV and proportion of abnormal were compared among the three groups.The subjects were divided into two group according to the ba-PWV:normal group and abnormal group.Hs-CRP of two groups were compared.The influencing factors of ba-PWV were analyzed by multivariate logistic regression.Results Ba-PWV of subjects in＜1 mg/L group,＜3 mg/L group and≥3 mg/Lgroup were (1 592.9±268.5)cm/s,(1 659.1±292.1)cm/s and (1 649.5±314.7)cm/s,there was no statistically significant difference among the three groups(F=2.896,P=0.056).The proportion of abnormal ba-PWV among those three groups were 72.0%,81.2% and 81.3%,χ2=6.821,P=0.033.The proportion of abnormal ba-PWV of ＜3mg/L group and ≥3 mg/L group were higher than ＜1mg/L group,P＜0.017.The hs-CRP of normal group and abnormal group were (-0.26±1.00)mg/L and (-0.06±1.04)mg/L respectively,t=1.990,P＜0.05.Logistic regression analysis showed that hs-CRP was not an independent factor affecting ba-PWV,while age,systolic blood pressure,triglyceride,fasting blood glucose and HbA1c were independent factors affecting ba-PWV.Conclusion The factors affecting ba-PWV include age,trisylglycerin and blood glucose level,while hs-CRP is not an independent factor affecting ba-PWV. |
