| 根据新诊断2型糖尿病的临床特点,梳理中医学新诊断2型糖尿病的相关理论,认为新诊断2型糖尿病更加符合中医“脾瘅”范畴。湿邪是重要病理因素,湿郁化热为发病的关键,根据湿热邪气主要累及的脏腑分为上焦肺卫湿热、中焦脾胃湿热、下焦肝胆湿热和湿热下注三种证型,此外,临床还可见寒湿内蕴证。治疗上主张以清热利湿为原则,再酌情配合芳香化湿、苦燥除湿、温化寒湿和淡渗利湿不同的祛湿方法。 |

| According to the clinical characteristics of newly diagnosed type 2 diabetes,the relevant Chinese medicine theories of newly diagnosed type 2 diabetes mellitus were reviewed;we think that newly diagnosed type 2 diabetes mellitus is more consistent with the category of spleen illusion in TCM.The dampness pathogen is a crucial pathogenetic factor and dampness stagnancy creating pathogenetic fire is the key to onset.According to the main zang-fu organs involved by the dampness-heat evil energy,it is divided into three syndromes:dampness-heat of lung-defense in upper energizer,dampness-heat of spleen and stomach in middle energizer,dampness-heat of liver and gall and downward diffusion of dampness-heat in lower energizer.In addition,internal retention of cold-dampness could also exist.In treatment,it is suggested that the principle is to clear heat and promote diuresis,supplementing different methods of eliminating dampness pathogen such as resolving dampness with aromatics,eliminating dampness by bitter,warming cold-dampness and promoting diuresis with drugs of tasteless flavor. |
